DEFINITION
Under administrative direction, to plan, organize, direct and evaluate the operations of the Care Connect. The Whole Person Care Pilot is performance-based, and draws funding based on achievement of objectives. Care Connect is a system transformation project, designed to improve care for individuals and families experiencing homelessness and/or repeated crises. The program creates data exchange, housing and human infrastructure to support members of the focus population in attaining their optimal level of health and stability, while reducing inefficiencies and unnecessary costs. DISTINGUISHING FEATURES This single position classification is located in the Health Care Services Agency Administration and Indigent Health’s Care Connect program. This position will oversee the design of and facilitate development of a new multi-system organizational structure that will support successful implementation of the Care Connect system. Incumbent is responsible for building relationships at multiple levels (from providers to executive leaders) among different systems, including healthcare delivery and payment, mental health and substance use, social services, criminal justice and probation, to improve cross-system coordination, integration and linkage. This position reports directly to the HCSA, Medical Director, who is responsible for overall implementation and operations of the Care Connect program. |
EXAMPLES OF DUTIES

Lead the establishment and management of the Care Connect backbone organization, ensuring effective coordination and communication within the backbone team and guiding the successful implementation and operations of the project. 2. Supervise the development and implementation of the overall project work plan and accountability framework and maintain a broad overview of complex interlocking program components spanning multiple large systems. Ensure that deliverables are clearly communicated to responsible participants throughout the many systems that are part of the Care Connect project. 3. Responsible for developing and supporting the Care Connect Steering Committee, an oversight and advisory body that provides input to the program from executive level leaders from significant stakeholder organizations and individuals. 4. Oversee tracking & delivering on programmatic requirements in order to maximize federal drawdown. Oversee contracts, ensuring that deliverables are regularly monitored and risks of non-achievement are managed and communicated. Ensure readiness for future financial and programmatic audits. 5. Oversee development of internal and external communication plan to ensure successes and lessons learned are gathered and shared, and to support understanding and build strong relationships both within the backbone organization and across multiple systems and organizations. 6. Responsible for generating accurate and comprehensive reports to the Department of Health Care Services (DHCS) in order to draw down Whole Person Care funds. 7. Represent County in discussions with DHCS regarding interpretation of regulations and program guidelines. Ensure program is in compliance with Whole Person Care, State and Federal regulations. 8. Provide direction related to program fiscal management and interface with County fiscal and administrative leaders to ensure complex funding flows, budgets and reporting are managed effectively and accurately. 9. Ensure advance planning for future sustainability of Care Connect services and infrastructure, including leading negotiations with major institutional partners regarding contributing resources to sustain improvements. 10. Prepare reports to the Board, Care Connect Steering Committee and other critical decision-makers. |
MINIMUM QUALIFICATIONS
|
Education:
Possession of a Master’s degree from an accredited college or university in social welfare, planning, public health, public administration, public policy, health care administration, social science or a closely related field. AND Experience: The equivalent of five years of full-time professional executive, administrator, or divisional leadership level experience in the evaluation of, or planning for, the delivery of health care or social services in a health care agency, social services agency, or a community organization. License: Possession of a valid California Driver’s License. NOTE: The Civil Service Commission may modify the above Minimum Qualifications in the announcement of an examination. |
