DESCRIPTION

Under general direction, collaborates with the executive management of Health Care Services Agency (HCSA), and their county partners from multiple departments, as well as community-based organizations. Gives advice on strategic matters. Designs new processes and models to facilitate strategic system development decisions. Translates strategies and policies into improved/changed processes, new guidelines, standards and models. Provides high level interpretation of rules and regulations. Works collaboratively with multiple stakeholders to improve the health and human condition of the medically fragile and marginally housed residents of Alameda County. Constructs and implements strategic plans for multiple countywide projects. Performs other work as required.  

DISTINGUISHING FEATURES  

This is a single position classification found only in the Health Care Services Agency Administration’s Indigent Health’s Care Connect program and reports directly to the Care Connect Director. This position is distinguished by its overall responsibility to collaborate with cross-system stakeholders to facilitate process and model redesigns to for system development and implementation process for Whole Person Care.


EXAMPLES OF DUTIES

NOTE:   The following are the duties performed by employees in this classification.  However, employees may perform other related duties at an equivalent level.  Each individual in the classification does not necessarily perform all duties listed.

  1. Identifies and surfaces the behavioral and social service intervention opportunities across the scope of the system re-design.  
  2. Participates at a leadership level in identifying the multiple systems that HCSA patients and their support network touch, surfaces the clinical psychosocial intervention opportunities and implications of system re-engineering for a range of medical and behavioral health care, social service, law enforcement and other county services.  
  3. May lead a team of specialists in progressing the detail of system re-design processes and projects.  
  4. Takes the lead in process improvement efforts and ensures a team is in place for the testing and implementation phases.      
  5. Persuades senior managers of the benefits of change in situations where there is significant reluctance, and likely disruption to the usual mode and method of doing business.    
  6. Provides consultation and training on clinical case management methods, process improvements, particularly psycho/social interventions for medically complex indigent individuals and their families.  
  7. Provides psychosocial clinical and policy expertise for design and implementation of the range of system change and organization development aspects of the project.   
  8. Interprets state and federal regulations and their impact on Care Connect’s current projects as well as the future health and human service systems in Alameda County.  
  9. Collaborates with clinical leadership across agencies to coordinate and integrate multidisciplinary services for clients with complex psychiatric, medical, and daily living problems.  
  10. Develop and maintain the consumer and family engagement strategy, including the design and implementation of consumer portals to inform system level service development and delivery challenges.  
  11. Provides clinical consultation and develops working relationships with other community agencies, collateral persons and others involved with the health populations served.  
  12. Leads in creation of intellectual agendas for culturally affirmative practice at direct service, management and leadership levels of systems.  
  13. Assesses for and designs clinical trainings to standardize skill of psychosocial care management for LCSW and allied staff across county agencies.  
  14. Creates clinic protocols and standardized procedures for new systems of care. Provides consultation and training on clinical case management methods and organizational development.  
  15. Translates critical consumer feedback into strategic change proposals and program redesign  
  16. Provide additional support to the Executive Director and Deputy Director as requested.

MINIMUM QUALIFICATIONS

Education  

A Master’s degree in Social Work or a related field.

AND

Experience  

Five or more years’ professional-level experience providing services in a clinical or social service setting, one year of which must have included experience working in integrated care delivery among physical health, mental health, homeless populations, substance abuse, and/or social supports such as housing connection.
