DESCRIPTION

Under direction works closely with the array of Care Connect psycho-social sector partners, particularly social services, health care and Alameda County Behavioral Health, to build the interface and institutionalize the collaboration between the various entities, build the clinical and psycho-social skills of their respective staff, and increase the quality and seamlessness of both the crisis and planned service flow of their common consumer and their families, with a focus on Care Connect consumers: people experiencing homelessness and frequent users of multiple systems.  

DISTINGUISHING FEATURES  

This single position classification is found only in the Health Care Services Agency (HCSA) Administration’s Indigent Health’s Care Connect program and reports directly to the Director of Clinical Case Management Methods. This position is distinguished by its responsibility to bring together stakeholders from county health and human service systems, particularly community medical and psychiatric services, other critical service providers and consumers, in a consultative and collaborative process to clarify gaps and identify best practice solutions and training needs, in order to better coordinate care and seamless support for Care Connect beneficiaries.


EXAMPLES OF DUTIES

NOTE:   The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.  

 

  1. In direct collaboration with the Director of Clinical Case Management Methods develop and maintain key components of the consumer and family engagement strategy, including the design and implementation of consumer portals to inform system level service development and delivery challenges.  
  2. In collaboration with senior management, design and define scope of a variety of training of psychosocial methods and pilots that meet the needs of individuals and families of Alameda County’s most vulnerable populations.
  3. In direct collaboration with the Director of Clinical Case Management Methods, lead the development of inter-agency provider partnerships that facilitates independent networking and collaborations; acquaintance and relationship with one another and their respective services, setting a common agenda, and addressing system level challenges.
  4. Assess current competencies of HCSA Care Connect Partners line staff psychosocial service providers.  
  5. Develop and facilitate cross discipline skill development for health and human service providers.  
  6. Provide direction, training and support to consumer activist in their role in providing feedback and consultation in system design.
  7. Create and deliver training modules that improve the culturally affirmative psychosocial assessment, engagement and interventions of direct service providers and the support provided to them by their management.
  8. Develop and oversee new academic partnerships with colleges and universities to establish Care Connect and Health Care Services Agency as a dynamic field placement, to create a multicultural workforce pipeline that possess the ethics, knowledge and skills to meet the health and human service needs of Alameda County’s most vulnerable residents.  
  9. Develop and provide consultation for clinical and operational Culturally Affirmative case management strategies, Care Coordination, and workflow, and assessment and engagement of consumer’s support network.
  10. Participate in the design of training for frontline providers on how to successfully engage and access critical clinical and community services for their clients, leads iterative improvement cycles to test and strengthen the workflow of the care system, leads data-driven evaluation of progress of this work, and drives resolution of system level challenges uncovered by this reorganization effort.
  11. Liaison with County agencies and foster cross sector collaboration and partnership.  
  12. Provide additional support to the Director of Clinical Case Management Methods as requested.  
  13. Lead Care Connect representation and input at allied services throughout Alameda County.

MINIMUM QUALIFICATIONS

Experience:  

The equivalent of five or more years of professional-level experience providing services in a clinical or social service setting, preferably in Alameda County.

 AND  

License:  

Possession of a valid license from the State of California Board of Behavioral Sciences.
