DESCRIPTION
Under general direction, plans, develops, organizes, and directs specialized projects and activities aimed at improving community health services and the Health Care Services Agency (HCSA) diversity programs in Alameda County; conducts community needs analyses and operational studies. The Community Health Services Coordinator is responsible for coordinating and overseeing community-building activities, community grant programs, and community health services volunteers; and perform related duties as required.

DISTINGUISHING FEATURES

This single position class is located in the Public Health Department and reports to the Director of Public Health.

This class is distinguished from the lower class of Program Specialist in that the latter class emphasizes direct program review and evaluation in the direct delivery of health care services to clients, whereas the Community Health Services Coordinator plans, coordinates and evaluates a variety of community activities and projects designed to enhance and expand health services and the HCSA diversity programs in Alameda County.

EXAMPLES OF DUTIES
NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.

1. Plans, develops and coordinates specialized projects and activities aimed at enhancing and expanding community health services and HCSA diversity programs in Alameda County.

2. Conducts studies involving assessment of community health and social needs; and formulates strategies, goals and objectives to meet the identified needs.

3. Prepares and presents reports with recommendations and appropriate justification based on community studies and surveys; and coordinates the implementation of new systems and/or procedures.

4. Facilitates and represents the Alameda County Public Health Department in a variety of community meetings and committees (e.g. Alameda County Public Health Commissioners, MAPP Core Committee); and develop alliances with community leaders.

5. Coordinates the House Calls Volunteer Program to gather community feedback on public health services; and participates in community focus groups (e.g. East Oakland Focus Group).

6. Coordinates and administers the County Community Mini-Grant Program; defines policies and procedures, establishes award criteria, and reviews proposals; and recruits applicants and awards grants.

7. Recruits, trains and supervises volunteers assigned to community-building activities.

8. Develops and reviews processes and negotiates contracts for the provision of expanding community health services and preventing community violence.

9. Interprets and implements State and Federal program mandates related to public health codes.

10. Serves as liaison with the Alameda County Public Health Commission, and interacts with other divisions of the Health Services Department, local, State, and Federal agencies, community leaders, and the community; provides training and technical assistance.

11. Prepares a variety of correspondence, periodic and special reports, informational publications, program documentation, policies, procedures and other written materials.

12. Operates a variety of standard office equipment including a word processor and/or computer; may drive a County or personal vehicle to make site visits and attend meetings.

MINIMUM QUALIFICATIONS
Either I
Education:

Possession of a Bachelors Degree from an accredited college or university with major coursework in business, public administration, education, a health or social science or closely related field.
AND
Experience:

The equivalent of five years full-time professional level experience in the coordination, management, strategic planning and evaluation of community-building activities and projects aimed at expanding and enhancing health services in a public health care setting.

(A Master’s degree from an accredited college or university with major coursework in business, public administration, education, a health or social science or related field may be substituted for two years of experience.)
