DESCRIPTION
|
Under administrative direction, leads the development and integration of programs focused on health equity and social determinants of health interventions; works closely with county government and community healthcare partners to develop, innovate, and implement programs and policies that will increase the overall health and well-being of Alameda County residents; works closely with health insurers, community clinics, community based organizations, philanthropy, business, academics, and county officials to ensure that effective and measurable strategies are implemented across the county; provides the blueprint for infrastructure needed to educate and innovate; and to do related work as required.
DISTINGUISHING FEATURES
This is a single classification currently located in the Alameda County ALL IN program and reports to the Executive Director of the ALL IN program overseeing all medical aspects within the ALL IN County program. This class is distinguished from the comparable Physician V classification in the Health Care Services Agency in that the ALL IN Physician is responsible for promoting well-being health equity and eliminating health care inequities within the ALL IN program while the Physician V is responsible for managing the overall medical services of a division/department within the Health Care Services Agency.

EXAMPLES OF DUTIES
|
NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.
- Provides leadership to elevate, innovate and integrate health equity, social determinants of health, and quality improvement principles and practices in the overall implementation of ALL IN programs that address poverty.
- Exercises leadership in state and county-wide interdisciplinary approaches to health equity and determinant of health innovation and integration. Develops and convenes partnerships and negotiates contracts with health systems and clinic networks, health plans, CBOs, county and state agencies, community champions, and place-based institutions (schools, workplaces, etc.) to innovate, develop, integrate, and implement successful health equity and determinant of health programs that promote overall health and well-being.
- Works closely with the Executive Director to develop and expand the influence and impact of ALL IN Alameda County by strategically partnering with state, county, and community organizations to innovate and develop anti-poverty programs that optimize both population and community health and well-being.
- Leads a variety of efforts focused on securing funding for innovative, health equity and social determinant of health programs.
- Represents ALL IN in partnerships with external entities in work related to Food as Medicine and other health equity and determinant of health programming.
- Leads the development and implementation of new and innovative clinical initiatives and programs that promote well-being, and address health equity and determinant of health integration. Develops and promotes an integrated health equity, determinant of health, and Food as Medicine model which can be replicated county wide and includes food pharmacies, behavioral pharmacies and social needs pharmacies. Works closely with ALL IN and the Center for Healthy Schools and Communities on implementation both at primary clinic sites and school-based health centers.
- Develops and partners with experts to implement training curriculums to onboard clinics across the county in the implementation of health equity, determinant of health, and Food as Medicine programs.
- Maintains clinical and programmatic expertise on an as-needed basis to inform all aspects of health equity and determinants of health programming. Serves as a liaison between health practitioners and the ALL IN team and provides on the ground support to clinics launching health equity and determinant of health programs to support community health and well-being.
- Strategizes, supports, and assists with clinical informatics work and health information exchange work to integrate health equity and social determinants of health screening and navigation services into electronic medical records and community health records.
- Develops a data and evaluation framework to track and measure health equity and determinants of health interventions and impact.
- Supports health equity, social determinants of health, and Food as Medicine data governance activities in Alameda County.
- Works closely with researchers locally and nationally to help move the agenda forward on health equity, social determinants of health, and Food as Medicine program integration.
- Develops and implements a policy agenda that can be used on a statewide basis to advocate for sustainable funding for health equity and determinant of health programs.
- Develops relationships with health insurers, statewide agencies, and sites throughout the state and nation that are implementing health equity, determinants of health, and Food as Medicine programs to advocate for policy change.
- Communicates with the Board of Supervisors and the County Administrator regarding policy opportunities.
- May act as the Executive Director in his/her absence.

MINIMUM QUALIFICATIONS
|

Experience:
The equivalent of four (4) years of post-residence experience which must have included at least two years of experience with supervisory and administrative responsibility.
AND
License:
Possession of a valid license to practice medicine in the State of California.
Possession of a valid California Motor Vehicle Operator’s license.
