1. A DMV printout showing current status of their Class A Commercial Driver's Licenseand proof of a clean driving record for the past eighteen (18) months. The DMV printout must be dated within the following period (Wednesday, May 10, 2017 through Monday, June 5, 2017).
2. A Tanks (N) Endorsement
3. A valid Medical Certificate
