Do you possess one year of STD-related work experience required for this position?

Note: STD services may include counseling, education and prevention, outreach, program management, training, community capacity building, direct patient services, or policy development. HIV experience is also qualifying.

Please describe your professional-level or supervisory work experience in the direct delivery of STD services to the community.  In your response, please include the name and description of the organization, your employment dates, your job title, and the scope/level of your responsibilities.

Describe your experience planning, administering and evaluating STD service programs. In your response, please detail the target population, CBOs or other community partners involved, and how you ensured programmatic goals were met.

Describe a problem you encountered in the service delivery and/or coordination of STD services.  Clearly state the problem, the resolution, and what factors you took into consideration when making your decision.

Please provide a specific example of a time in which you participated in or led a community capacity building project that involved multiple stakeholders, including CBOs, community members, or other groups.  Please describe the situation, your efforts to engage other groups, and the outcome.

Describe your experience gathering and synthesizing data from multiple sources. In your response, please detail the methods and sources you used, how you summarized and/or interpreted the data, and how the data was used.
