All Candidates must submit evidence from DMV showing the current status of their driving record and proof of having a clean driving record for the previous three (3) years.  Please attach a scanned copy of your DMV printout to your on-line application under the RESUME TAB.  The DMV printout must be dated during the period of November 25, 2019 to December 20, 2019. Please scan all related documents and upload one (1) file with your application and not multiple files.   

Note:  To upload your DMV printout, follow the instructions located on the Resume Tab of your application.  The Resume Tab is located at the top of the screen after you have completed this supplemental questionnaire.  Please remember to upload ONE (1) document with all your files and NOT multiple files of your document.
