In the role of Family Support Case Manager with the MPCAH unit, you will be working with perinatal populations and clients with a variety of medical and social risk factors. Please describe the training, education and experience you possess which has prepared you to successfully perform the duties of a Family Support Case Manager with the MPCAH unit. In addition to describing work with individuals, include any group-level services experience you may have. In your response, please describe why you are interested in this particular position.

Visiting clients in their homes, often times in low-income neighborhoods, is an essential function for Family Support Case Managers. Please provide an example of a time when you had to visit a client’s home, or arrange a meeting elsewhere in the community, in order to assist a client. In your response, please include the following: your role and the reason for the visit, the action(s) you took during and after the visit, and the outcome. Why was this an important intervention?

Describe your experience working in or with diverse, multi-ethnic, multi-cultural communities. What have you done to establish and maintain your rapport and working relationships with these communities?

Family Support Case Managers work with clients affected by a variety of social risk factors, often times in high stress situations. Provide a specific example of a conflict you experienced with a client you were trying to assist. What was the situation? What was the outcome? What tools have you utilized to help alleviate conflict in your work?

Describe the experience you possess conducting outreach activities for the purpose of recruiting clients for case management programs and educational events. In your response, please address your approach to planning, organizing and implementing successful community-wide events throughout program target areas and documenting your outreach successes.
