Thank you for your interest in the position of Community Outreach Worker II.  Your completed response to this supplemental questionnaire will be evaluated to determine your qualifications and must be completed properly in order to be given full consideration for the next phase in the selection process.  Responses should be thorough and specific, yet concise and succinct.  A lack of adequate detail in the supplemental questions and in your application may result in failure or disqualification for this recruitment.  Neatness, clarity of expression, grammar, spelling and the ability to follow instructions will be considered in the evaluation process.  A resume will not be accepted as a substitute for a thoroughly completed employment history and supplemental responses.

Question 1 and 2 will be reviewed to further verify the minimum qualifications:

Do you possess the equivalent of six months of full-time satisfactory experience in the Community Outreach Worker I in the Alameda County classified service?

Do you possess two years’ full-time experience working in a community outreach program performing duties, such as: Intake, peer counseling, obtaining personal history data needs, non-clinical assessments, client community health education, program screening, placement and referral.

Questions 3 thru 5 will be used to screen for the best qualified component.

Describe how you build and maintain effective working relationships with community-based organizations or government agencies, and how you connect those organizations’ resources and services with the needs of the clients you serve.

When working with clients, what are some strategies you use to acknowledge and accept different attitudes clients may possess due to facing financial and/or medical challenges?

Describe your experience working supportively with clients and families from diverse ethnic and cultural backgrounds, in order to gather accurate personal information.
