Thank you for your interest in the position of Food Service Worker. Your completed response to this supplemental questionnaire will be evaluated to determine your qualifications and must be completed properly in order to be given full consideration for the next phase in the selection process. Additionally, your responses will also be evaluated and used in the selection process in order to identify the best qualified applicants.

Responses should be specific, thorough and succinct.  A lack of adequate detail in the supplemental questions and in your application may result in failure or disqualification for this recruitment.  Clarity of expression, grammar, spelling and the ability to follow instructions will be considered in the evaluation process.  A resume will not be accepted as a substitute for a thoroughly completed employment history and supplemental responses.

This position is located in the Juvenile Justice Center, a 24-hour Probation facility. Do you have any experience working in a correctional facility (in any job), or a juvenile detention facility?

Describe your most recent work experience where you either served or assisted in the preparation of food or washing dishes, pots and pans in a large cafeteria or dining room. (Large scale food operations may include but are not limited to hospitals, schools, prisons, nursing homes, institutions, restaurants, or military units in which at least 200 complete meals are prepared each day.) Describe your work assignments, the employer, the average number of people daily served.

Describe your specific work experience assisting in the preparation, receiving, storing, packaging, and assembling of meals, as well as, setting up food carts and assisting with food service.
