Introduction

THIS IS A NEW ASSEMBLED EXAMINATION.  The eligible list resulting from this examination will cancel any existing list and may last approximately one year but can be extended.

Supplemental Questionnaire: A properly completed Supplemental Questionnaire must be submitted with each application. Applications and Supplemental Questionnaires must be in the possession of the Human Resource Services Department by 5:00 p.m. on the Last Day for Filing.  Failure to submit the Application or Supplemental Questionnaire will result in disqualification.


Qualified bilingual persons who speak English and are also fluent in: Spanish, Cantonese, and Mandarin are especially encouraged to apply. There is an additional bi-weekly compensation for persons in positions designated bilingual. Qualified candidates may be tested to demonstrate language proficiency.

DESCRIPTION

ALAMEDA COUNTY HEALTH

Alameda County Health is the local government agency that promotes and protects the health and well-being of all who live, work, learn, and play in Alameda County. We coordinate services and cultivate partnerships with community organizations and providers to help ensure access, organize, and deliver health care and services to people with Medi-Cal and without insurance, support resilient communities, and improve health for all. We focus on health equity by developing programs and systemic solutions that reduce disparities for the people and communities we serve.

Alameda County Health’s departments and programs focus on services and support that provide care for the whole person.

· The Behavioral Health Department provides mental health and substance use services for people with Medi-Cal and without insurance and supports people along their path to wellness, recovery, and resilience.

· The Environmental Health Department works to keep our air, water, and food safe; it regulates, protects, and promotes the health of everyone in Alameda County by enforcing environmental health codes to reduce exposure to toxins and diseases.

· The Public Health Department focuses on community and population-level health, preventing and addressing root causes of health inequity across a range of communicable and chronic diseases.

· Alameda County Health also provides services through Housing and Homelessness Services, Emergency Medical Services Agency, HealthPAC, and Healthy Schools and Communities.

PUBLIC HEALTH DEPARTMENT

As part of Alameda County Health, the Public Health Department works in partnership with our local communities to ensure the optimal health and well-being of all people. We monitor health status and service delivery, prevent disease, mobilize communities, conduct outreach, and promote health policy and education. We actively seek partnerships and engage with the community to improve community health through a dynamic and responsive process that respects diversity, addresses health equity, and challenges us to provide for present and future generations.

We are nurses, doctors, community psychologists, epidemiologists, dentists, medical social workers, physical and occupational therapists, dietitians, outreach workers, health educators, program managers, and pre-hospital care coordinators.

THE POSITION
Under general supervision, to perform specific program care coordination, group facilitation, and a variety of community and health education activities in the Health Care Agency (HCSA) case management, care coordination, home visiting and/or family support programs and work collaboratively to provide a seamless system of services and support for clients in Alameda County; acts as a liaison between communities, agencies, and other resources, services and program staff; and to do related work as required. Care coordination is defined as brief screening, triage, and referral/linkages to other programs services that occurs over the course of a six-month to one-year period of time with a client/family.

  • Ability to build and maintain professional, constructive and collaborative relationships with diverse communities, organizations, and local public and private service providers.
  • Exceptional written, verbal and interpersonal communication skills, focused on developing an environment where diverse multidisciplinary teams can work together to offer maximum services to Alameda County residents.
  • Experience interacting with clients and peers in a way that gives them confidence in your intentions and those of the organization.
  • Actively seeks information to understand client circumstances, problems, expectations and needs, in order to provide clients with appropriate education, care, and service referrals.
  • Ability to effectively manage time, resources and priorities – identifying more critical and less critical activities and tasks and adjusting priorities as appropriate.
  • Knowledge of, and familiarity with, Windows based computer programs.
 
THE VACANCY
The current vacancies are located in the Public Health Department. However, the eligible list resulting from this recruitment may be used to fill future vacancies in other divisions or program areas.

MINIMUM QUALIFICATIONS

Either I
Pattern I

Two (2) years as a Community Outreach Worker or an equivalent or higher-level classification performing care coordination/case management duties.

Or
Pattern II

Possession of an AA degree and one (1) year of experience performing care coordination/case management duties in a health care or social services setting.

NOTE: The Civil Service Commission may modify the above minimum qualifications in the announcements of an examination.

KNOWLEDGE AND SKILLS

The most suitably qualified candidates will possess the following competencies:

Knowledge of:
  • Extensive knowledge of community and governmental services and resources
  • Interest in working with multi-stressed and multicultural families and the communities in which they live.
  • Experience observing clients, recording information, conducting client interviews, implementing service plans
  • Problem-solving techniques, handling crisis intervention matters, and using proper decision making skills
  • Effective communication and interpersonal skills, including reflective practice
  • A sense of responsibility and the ability to manage time effectively.
  • Knowledge of and experience with interviewing techniques and methods including Motivational Interviewing.
  • Knowledge of Ten Essential Public Health Services
  • Understanding of basic social needs, attitudes and behavioral patterns; the principles of counseling and health education, health promotion, disease prevention, and preventive health care.
  • A strong desire to help others and the ability to establish trusting relationships
  • Knowledge of community structures and dynamics and method of group facilitation.
  • Outreach experience and knowledge of, program development, implementation and evaluation.
  • Knowledge of computer database operation and basic data entry skills.

Ability to:
  • Establish and maintain effective working relationships with clients, their families, professionals, para-professional and support staff in the department, outside agencies, schools and with the general public in a variety of ethnic and cultural communities.
  • Communicate clearly and effectively, both orally and in writing with linguistic and cultural proficiency.
  • Follow prescribed procedures and policies.
  • Obtain and maintain accurate case information in electronic database format.
  • Understand and accept differences in attitudes toward health problems resulting from medical, cultural, socioeconomic and other factors
  • Understand the connections between social conditions (income/community economics, transportation, education, housing, incarceration) and individual and community health
  • Use knowledge to assist clients in accessing resources that address barriers
  • Analyze client activities and integrate appropriate program activities/services.
  • Establish trust with families in order to obtain accurate personal, sensitive and confidential data from clients across ethnic and cultural lines.
  • Recognize the contributions of diverse opinions and perspectives
  • Incorporate ethical standards of practice into all interactions with individuals, organizations and communities.
  • Communicate information to influence behavior and improve health

EXAMINATION COMPONENTS

THE EXAMINATION WILL CONSIST OF THE FOLLOWING STEPS:

  1. A review of applicants' applications to verify possession of minimum requirements. Those applicants who possess the minimum requirements for the class will move on to the next step in the examination process.
  2. A review of candidates’ Supplemental Questionnaires to determine the best qualified. Those candidates considered the best qualified will move on to the final step in the examination process.
  3. An oral interview which will be weighted as 100 % of the candidate's final examination score. The oral interview may contain situational exercises.
