ALAMEDA COUNTY HEALTH

Alameda County Health is the local government agency that promotes and protects the health and well-being of all who live, work, learn, and play in Alameda County. We coordinate services and cultivate partnerships with community organizations and providers to help ensure access, organize, and deliver health care and services to people with Medi-Cal and without insurance, support resilient communities, and improve health for all. We focus on health equity by developing programs and systemic solutions that reduce disparities for the people and communities we serve.

Alameda County Health’s departments and programs focus on services and support that provide care for the whole person.

· The Behavioral Health Department provides mental health and substance use services for people with Medi-Cal and without insurance and supports people along their path to wellness, recovery, and resilience.

· The Environmental Health Department works to keep our air, water, and food safe; it regulates, protects, and promotes the health of everyone in Alameda County by enforcing environmental health codes to reduce exposure to toxins and diseases.

· The Public Health Department focuses on community and population-level health, preventing and addressing root causes of health inequity across a range of communicable and chronic diseases.

· Alameda County Health also provides services through Housing and Homelessness Services, Emergency Medical Services Agency, HealthPAC, and Healthy Schools and Communities.



PUBLIC HEALTH DEPARTMENT

As part of Alameda County Health, the Public Health Department works in partnership with our local communities to ensure the optimal health and well-being of all people. We monitor health status and service delivery, prevent disease, mobilize communities, conduct outreach, and promote health policy and education. We actively seek partnerships and engage with the community to improve community health through a dynamic and responsive process that respects diversity, addresses health equity, and challenges us to provide for present and future generations.

We are nurses, doctors, community psychologists, epidemiologists, dentists, medical social workers, physical and occupational therapists, dietitians, outreach workers, health educators, program managers, and pre-hospital care coordinators.

THE POSITION

Under general supervision, to perform program-specific, evidence-based, long-term case management in the Health Care Services Agency (HCSA) case management, care coordination, home visiting and/or family support programs and work collaboratively to provide a seamless system of services and support for clients in Alameda County; acts as a liaison between communities, agencies, and other resources, services and program staff; and to do related work as required. Program-specific, evidence-based case management is defined as intensive work with families over a period of two to three years using evidence-based models specific to the program which incumbent is assigned and target client population(s). These programs are proven to improve health and psychosocial status of clients served.

THE VACANCY
There is currently one vacancy located in the Health Families America program within the Maternal, Paternal, Child and Adolescent Health (MPCAH) unit of the Public Health Department. However, the eligible list resulting from this recruitment may be used to fill future vacancies within other program areas.

MATERNAL, PATERNAL, CHILD, AND ADOLESCENT HEALTH (MPCAH) UNIT
Maternal, Paternal, Child, and Adolescent Health (MPCAH) is dedicated to coordinating services for families, including teens, pregnant women, mothers, fathers, caregivers and children in Alameda County.  MPCAH enhances access to comprehensive, quality health care and focuses on early intervention and prevention services.  The goal is to reduce health disparities, protect and improve health outcomes among Alameda County families, including pregnant women, parenting women and men, and their children.

MPCAH programs work to ensure that women, children, youth, fathers, and families achieve optimal health and well-being through the provision of client-centered, culturally responsive, strengths-based services and supports, combined with community transformation efforts that aim to improve neighborhood conditions.

HEALTHY FAMILIES ALAMEDA COUNTY
Healthy Families Alameda County is a voluntary home visitation program designed to promote family and child health and wellbeing through a variety of services, including child development, access to health care, and parent education. The program targets at risk pregnant women and families with newborns under 2 weeks of age. Services are provided until the child reaches 3 years. One of the two positions will be serving Spanish speaking families.

THE IDEAL CANDIDATE
In addition to meeting the minimum qualifications below, the ideal candidate will have experience conducting in home and community visits with clients, conducting outreach, administering screenings and assessments, and maintaining timely and accurate documentation related to client needs and services provided. Additionally, the ideal candidate will possess the following critical attributes:

  • Experience using a collaborative process to assess, recommend, plan for and provide comprehensive case management in clients’ homes or in the community.
  • Exceptional written and verbal communication skills, and the ability to communicate effectively with diverse audiences and in a variety of contexts.
  • Eagerness to understand client needs – actively seeking information to understand client circumstances, problems, expectations, and needs and verifying understanding.
  • An understanding of the impact internal and external issues (e.g., economic, political, social trends) have on the work of the organization.
  • Ability to work effectively with individuals of diverse cultures, interpersonal styles, abilities, motivations or backgrounds.
  • Ability to build and maintain professional, constructive and collaborative relationships with diverse communities, organizations, and local public and private service providers.
  • Ability to generate relevant options for addressing problems/opportunities and achieving desired outcomes.
  • Ability to effectively manage conflict; taking positive action to resolve conflict in a way that addresses the issue, dissipates the conflict, and maintains the relationship.
  • Experience interacting with clients and peers in a way that gives them confidence your intentions and those of the organization.
  • Professional Integrity - displaying and promoting high standards of ethical conduct and behaviors consistent with agency and governmental standards.
  • Ability to remain self-reflective and open to suggestions and ideas.
  • Knowledge of, and familiarity with, Windows based computer programs and electronic case management systems.

MINIMUM QUALIFICATIONS

Possession of a Bachelor’s degree and three (3) years performing case management duties for clients with complicated medical and social risk factors in a health care or social services setting.

The most suitably qualified candidates will possess the following competencies:

  • Targeted Case Management Reporting (TCM)
  • Assessment skills, including facility with validated screening tools.
  • Extensive knowledge of community and governmental services and resources
  • Interest in working with multi-stressed and multicultural families and the communities in which they live.
  • Experience observing clients, recording information, conducting client interviews, implementing service plans

  • Problem-solving techniques, handling crisis intervention matters, and using proper decision making skills
  • A strong desire to help others and the ability to establish trusting relationships
  • Effective communication and interpersonal skills including reflective practice

  • Interviewing techniques and methods including motivational interviewing.
  • Understanding of basic social needs, attitudes and behavioral patterns; the principles of counseling and health education, health promotion, disease prevention, and preventive health care.
  • Community structures and dynamics and method of group facilitation.
  • Outreach experience and knowledge of, program development, implementation and evaluation.
  • Public Health funding mechanisms and compliance
  • Computer database operation and basic data entry skills.

Ability to:
  • Provide interventions to clients beyond supportive counseling and health education.
  • Chart all client interactions succinctly and within the charging guidelines provided by the evidence-based model.
  • Establish and maintain effective working relationships with clients, their families, professionals, para-professional and support staff in the department, outside agencies, schools and with the general public in a variety of ethnic and cultural communities.
  • Communicate clearly and effectively, both orally and in writing with linguistic and cultural proficiency.
  • Follow prescribed procedures and policies.
  • Obtain and maintain accurate case information in electronic database format.
  • Understand and accept differences in attitudes toward health problems resulting from medical, cultural, socioeconomic and other factors Analyze client activities and integrate appropriate program activities/services.
  • Understand the connections between social conditions (income/community economics, transportation, education, housing, incarceration) and individual and community health
  • Use knowledge to assist clients in accessing resources that address barriers.
  • Establish trust with families in order to obtain accurate personal, sensitive and confidential data from clients across ethnic and cultural lines.
  • Research, prepare and deliver preventive health presentations.

EXAMINATION COMPONENTS

THE EXAMINATION WILL CONSIST OF THE FOLLOWING STEPS:

  1. A review of applicants' applications to verify possession of minimum requirements. Those applicants who possess the minimum requirements for the class will move on to the next step in the examination process.
  2. A review of candidates’ Supplemental Questionnaires to determine the best qualified. Those candidates considered the best qualified will move on to the final step in the examination process.
  3. An oral interview which will be weighted as 100 % of the candidate's final examination score. The oral interview may contain situational exercises.
