DISCOVER THE OPPORTUNITY TO:
- Provide nursing services in the residence;
- Coordinate medication changes with RVS prescriber and outside providers;
- Provide medication education to clients and staff;
- Conduct assessment of medical needs and appropriate follow-up;
- Maintain strong communication with the Transitional Residence Program team;
- Responsible for the overall coordination related to the integration of behavioral health and physical services;
- Conduct comprehensive physical health screens; and
- Participate in the development of plans of care as part of a multi-disciplinary team and goals development for individuals with co-occurring chronic conditions.

RVS a community mental health center serving Middlesex County and the towns of Lyme and Old Lyme, is operated by DMHAS. RVS’s goal is to provide the highest quality crisis intervention, case management, and residential and clinical services to adult citizens, 18 years of age and older, who suffer from serious mental disorders and who lack the financial means to secure such services in the private sector.
Services are directed at improving clients’ quality of life and fostering dignity, ability, and recovery. ABOUT THE PROGRAM:
The Transitional Residential Program (TRP) is a residential program that provides 24/7 staffing and intense support services to 10 residents. The program serves individuals who are transitioning out of long term hospitalization or incarceration and are in need additional supports before transition into their community placements. Referrals for TRP come directly from DMHAS Office of the Medical Director.

PURPOSE OF JOB CLASS (NATURE OF WORK)
In a state agency this class is accountable for administrative and clinical supervision of professional nurses, paraprofessionals and other staff involving direct nursing care of patients/individuals or performing specialized professional nursing work in the areas of community or therapeutic nursing. At the State Universities or State Vocational Technical Schools this class is accountable for providing primary nursing care for students and facility staff.
EXAMPLES OF DUTIES
WORKING LEAD:
- Leads the planning of care and implementation of the nursing process;
- Plans unit workflow and determines priorities;
- Schedules, assigns, oversees and reviews work;
- Makes recommendations for personnel actions such as selection, discipline and performance; conducts or assist in conducting performance evaluations;
- Leads professional and paraprofessional nursing staff in provision of patient/individual care;
- Maintains and promotes standards of nursing;
- Ensures controlled drugs are properly stored, administered and ordered;
- Identifies staff development needs;
- Provides staff training and assistance;

DIRECT CARE:

- Maintains records and makes reports; provides for nursing services to comply with ICF regulations and JCAHO standards;

KNOWLEDGE, SKILL AND ABILITY

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE
Two (2) years of experience as a Registered Professional Nurse
MINIMUM QUALIFICATIONS - SUBSTITUTIONS ALLOWED
A Bachelor’s degree in Nursing may be substituted for one (1) year of the General Experience.
PREFERRED QUALIFICATIONS
- Experience working with individuals with complex psychiatric and medical conditions in an outpatient/community setting.
SPECIAL REQUIREMENTS
- Incumbents in this class must possess and retain a license or temporary permit as a Registered Professional Nurse in Connecticut.
- Incumbents in this class may be required to be fluent in a foreign language or proficient in the use of the American Sign Language in certain designated positions.
- Incumbents in this class may be required to travel.
CHARACTER REQUIREMENTS

WORKING CONDITIONS
