Summary Statement

The class incumbent is responsible for developing plans and administering current service provision to individuals with special healthcare needs, including the most complex, who live with their family or in community based settings.

Nature and Scope

The incumbent reports to a Division Director and administers a statewide program of services and supports to enable individuals with special healthcare needs, including the most complex, to live in the community as independently as possible. The program provides for an array of residential, day, and family support services designed to meet the needs of individuals with special healthcare needs. Work includes planning, organizing and directing, through subordinate managers, supervisors and professionals, a broad continuum of services including medical, behavioral, crisis prevention and intervention, rehabilitative, therapy, social and/or other related services to meet the needs of eligible consumers, overseeing and directing provider agencies who deliver community based services, and providing expert consultation and instruction in the development and implementation of individualized support plans. A class incumbent ensures compliance with all applicable Home and Community Based federal program requirements.

  • Directs home and community based services and supports for individuals with special healthcare needs, including the most complex, who are determined eligible for DHSS services; Develops and implements policies and procedures governing program operations.
     
  • Directs a unit of professionals who provide behavioral consultation to eligible consumers, along with transition coordination for individuals.
     
  • Provides oversight and direction to provider agencies that provide home and community based services.
     
  • Provides expert consultation and instruction in the development and implementation of individualized behavioral support plans.
     
  • Develops and administers budgets, grants and contracts involving state and federal funds; ensures the provision of quality programs through the proper allocation of resources.
     
  • Supervises case management staff in the development, implementation and monitoring of person-centered Plans of Care for each client enrolled in a DHSS Home or Community based federal program based on assessment of each individual’s strengths, preferences and support needs.
     
  • Ensures all community services staff providing direct services meet division training requirements.
     
  • Ensures the development and implementation of program and staff evaluations.
     
  • Participates on interdisciplinary team processes as necessary.
     
  • Represents Division on the Interagency Collaborative Team managed by the Department of Education.
     
  • Develops short and long range plans for the home and community based program and manages resources required to operate the program.
     
  • Establishes cooperative agreements with appropriate agencies and functions as a liaison with other agencies and programs.
     
  • Ensures that home and community based services are provided in compliance with appropriate state and federal standards and regulations.

  • Knowledge of theories, principles, practices and techniques used in the field of community health services administration and supervision over a broad continuum of services including medical, behavioral, crisis prevention and intervention, rehabilitative, therapy, social and other related services to meet the needs of eligible consumers.
  • Knowledge of theories and practices of various behavioral intervention strategies.
  • Knowledge of management theory and practice.
  • Knowledge of community based needs and services for individuals with special healthcare needs.
  • Knowledge of community, public, private, state and national health resources and program goals, objectives and funding sources.
  • Skill in the development of services, supports and plans for individuals with complex special healthcare needs.

  • Ability to supervise interdisciplinary staff who plan or deliver services to individuals with special healthcare needs.

Job Requirements

JOB REQUIREMENTS for Director DHSS Community Services
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
  1. Possession of a Masters' degree or higher in a Behavioral or Social Science or related field.
  2. Three years' experience in health or human services program administration such as overseeing and directing the development, implementation and evaluation of health or human services programs and services; planning and establishing short and long range program goals and objectives. Providing advice to other agency organizational units through consultation.
  3. Three years' experience in strategic planning such as planning and mapping a path between the present and future usually for three to five years by determining key objectives, how to accomplish the key objectives, what strategies should be used, what activities would contribute to accomplishing the key objectives and developing performance measures to gauge and report progress or success.
  4. Three years' experience in developing policies and procedures.
  5. Three years' experience in managing and negotiating the use of resources, such as grants, budgets and contracts to achieve programmatic or organizational goals, and evaluating the effectiveness of the delivery of those services.
  6. Three years' experience in operations management which includes planning, directing, coordinating, controlling and evaluating operations typically through subordinate supervisors.
