This position will provide person-centered case management support to individuals who are Deaf and receiving residential habilitation services. Duties include: the development, completion, and monitoring of individual service plans, communicating with service providers, families, guardians, and each individual’s informal support entities to enable the individual to live his/her best life, to ensure that services are being delivered in a manner that is consistent with the service plan, and to ensure compliance with applicable standards and regulations.
