Main Purpose of Job

The main purpose of this full time position will be to direct and oversee Care Coordination activities, including the Law Enforcement Assisted Diversion (LEAD) Program and Emergency Department Care Coordination (EDCC). This includes facilitation of teams, monitoring of programmatic data, reporting, and coordination with partners. Care Coordination is intended to support and empower participants by partnering with them to obtain needed community resources that will support their recovery, health, functionality, and well-being. This supervisor will be responsible for directing programmatic and administrative activities within the LEAD/EDCC Team. The desired candidate should possess good administrative leadership skills to operate as part of the Behavioral Health Leadership Team and aid in leadership functions and decisions of the Behavioral Health Unit. This position does include home visiting and may be required to work some evening and/or on call hours on a rotating basis. The selected applicant must be able to work and travel to all Worcester County Health Department sites.

MINIMUM QUALIFICATIONS

Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

Experience: Three years of professional experience in health services, two years of which must have been in the option for which application is made.

Notes: 1. Applicants may substitute a master’s degree from an accredited college or university in a health or human service field for one year of the required general experience.

2. Experience in the option must be as follows: professional clinical or administrative work in health or medical services in areas other than Mental Health, Developmental Disabilities or Addictions.

3. A bachelor’s degree from an accredited college or university in another field plus one additional year of professional experience in health services may be substituted for the specific degree.

4. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should have:
  • Knowledge of Statewide agencies, facilities, and community organizations providing services to individuals with an ability to network and partner. 
  • Knowledge and experience in high level communication with co-workers, management personnel, program participants, health care staff and the general public with practice experience in motivational interviewing. 
  • Ability to stimulate, grow and coordinate available inter-agency and community programs and services as the program is being newly developed. 
  • Experience with managing grants and working within a budget.
