POSITION DUTIES

The Coordinator of Special Programs II works for the Breast and Cervical Screening Program (BCCP), Colorectal Cancer Screening Program (CRCP), and Tobacco Prevention and Cessation Program (TPCP). This position coordinates assigned program planning and evaluation activities in accordance with agency operating policies and procedures. This position interviews and determines eligibility for individuals interested in the breast, cervical, and colorectal cancer screening programs or tobacco cessation program among Howard County and Maryland residents. Based on health history and client need, this position recommends the services available and acts as the liaison to community providers, ancillary service agencies, and health department staff; coordinates community health events to provide education and outreach to the Hispanic population and general public; provides Hispanic/English interpretation and translation assistance for persons receiving health services through this program. This position collects and reports cancer program data for program planning and evaluation.

This position is eligible for hybrid telework.

75% - Cancer Screening & Smoking Cessation Programs/Agency Liaison
  • Promotes and coordinates cancer screening and smoking cessation program activities and conducts education and outreach for the general public and Hispanic population.
  • Promotes preventive cancer screenings and smoking prevention/cessation through provider/health care professional partnerships.
  • Translates/interprets and facilitates communication between 
  • Hispanic clients with providers, ancillary services, and cancer program staff.
  • Assists clients with program intake and enrollment.
  • Schedules medical appointments/transportation and refers client for appropriate services.
  • In collaboration with licensed health care providers, serves as an associate case manager and patient navigator for clients enrolled in the cancer screening and smoking cessation programs or diagnosis and treatment program.
  • Seeks clinical consultation from the Cancer Program Nurse Supervisor, the Nurse Case Manager, and the Program Administrator, as needed.
  • Maintains and completes accurate client records to ensure proper documentation and care coordination according to program guidelines.
  • Maintains and tracks monthly referrals for smoking cessation.
  • Facilitates smoking cessation classes and counseling sessions.
  • Assist with facilitating Tobacco Awareness Program for Teens.
  • Enters client demographic, medical history, screening, and diagnostic information into the appropriate databases.

20% Community, Education, Outreach and Partnership Collaboration
  • Builds, collaborates, and maintains relationships among local providers, organizations and HCHD staff to identify target populations in the community and to promote preventative health cancer programs. 
  • Provides consultation to health department staff and care providers in the community on cultural issues affecting service to Hispanic clients.  
  • Assists in developing cultural competency training programs for the cancer program or other health department staff. 
  • Assists in maintaining a diverse group of contracted providers for cancer programs to promote options for Hispanic clients. 
  • Serves as liaison and educator with businesses, community service agencies, faith-based organizations, behavioral and health care settings, and schools, with focus on those that serve Hispanic populations.  
  • Works collaboratively with the nursing staff, ACCU, Behavioral Health, WIC, Program Supervisors, the Bureau Director and Medical Director to meet performance measures.
  • Plans, develops, and implements presentations to community agencies to describe available services.
  • Attends Cancer and Tobacco Coalition meetings as a representative of the Hispanic community, as needed.
  • Attends health fairs and community events to provide education, interpretation, and cultural sensitivity to the community and Hispanic population.
  • Tracks and reports statistics for MDH grant reporting and evaluation requirements.
  • Tracks and reports smoking education and cessation statistics to the Program Administrator for monthly County Executive and HealthStat Reports.

5% Other Duties as Assigned 
  • Accepts responsibility for special projects as assigned
  • Participates in Emergency Preparedness & Response exercises, drills and events.
  • Participates in seasonal flu clinics.

MINIMUM QUALIFICATIONS

Education:   A Bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

Experience: Two years of professional experience in health services, one year must have been in the Health Services option.

Notes:

1. Candidates may substitute the possession of a Master’s degree from an accredited college or university in a health or human service field for one year of the required general experience.

2. Experience in the Health Services option must be as follows: professional clinical or administrative work in health or medical services in areas other than Mental Health, Developmental Disabilities or Addictions.

3. Candidates may substitute the possession of a Bachelor’s degree from an accredited college or university in another field plus one additional year of professional experience in health services for the required education.

4. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

Desired qualifications: 
- Bilingual in Spanish
- Experience with conducting health outreach events
- Experience with Public Speaking
- Experience with customer service with patients, providers, and external agencies 
- Experience with scheduling appointments in a health setting 
- Experience navigating individuals through the system of care (including community providers and private insurance referrals)
- Extensive Knowledge and Experience using Microsoft Word, Excel, PowerPoint, Outlook
- Experience and Familiarity with OneDrive, Skype for Business, MS Teams, and WebEx
