Main Purpose of Job

This full-time merit position will enroll, provide case management and submit initial authorizations for Care Coordination funding for all Kent County residents. This position will assure that the Recovery In Motion Center (RIM) is providing equitable care and that overall program operations are adhered by. Other responsibilities include assisting in the preparedness and development of procedural manuals, statistical reports, work-flow charts, performance measurement criteria and/or organization charts; assisting in the formulation, establishment or modification of program goals and objectives per grant conditions of awards (COA); assist immediate supervisor in providing management oversight of any grant conditions of awards (COA) by reporting program performance and measurements; carry out the functions of the Overdose Fatality Review Coordinator (OFR) and facilitate quarterly meetings and complete necessary reporting; consult with others (community partners, family, KCBH staff) regarding consumers' recovery and aftercare plan; ensure RIM program is available to community members and community partners regarding information and resources for behavioral health use disorders persons, family members of persons, local businesses and be a resource for those in recovery to ensure community centers are open and available. This position will also assist with inventory, reviewing and creating requisitions for supplies and invoice payments.

This position is considered to be essential in times of emergency. Some on-call, evening and weekend hours per program needs.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.
