Main Purpose of Job

To provide professional Clinical Social Work services and Case Management to patients/residents and their families/significant others, guardians, and other surrogates of Western Maryland Hospital Center.

Per the Amended Directive and Order Regarding Vaccination (MDH # 2023.04.18.03) issued by the Secretary of the Maryland Department of Health, effective April 18, 2023, staff in all hospitals and residential facilities operated by the Maryland Department of Health or any local health department are required to show proof of COVID-19 vaccination. Staff includes, but is not limited to, regular and contractual employees, contractual staff, volunteers, and other state employees performing any duties at the facility. An individual may request an accommodation by providing appropriate and sufficient documentation for bona fide medical or religious reasons. 

POSITION DUTIES

Completes a comprehensive biopsychosocial-spiritual assessment through assessment of the patient/residents’, in collaboration with families, guardians or other identified support persons. Including the identification of patient/family/guardian goals, the internal/external strengths and barriers associated to reaching these goals. Development of Social Work interventions to support achieving goals. Identifies Assesses, facilitates, plans, evaluates, and coordinates healthcare services on behalf of the patient/resident throughout the patients/resident stay and coordinates a safe discharge plan with the treatment team. Participates in the interdisciplinary treatment team; completes and provides guidance to the utilization review process, care planning/discharge process and family conferences; reviews care plans with patients/residents and updates the patient/resident goals; conducts family tours and is a liaison for the facility and community partners. Make safety part of the job; monitor improvement of processes and explore ways the work environment can be safer.  Understand what a sentinel risk event is and how to report it. Uses Safety Alerts appropriately. Social Work Services provided in the nursing home level of care “shall provide or make arrangements for services to identify and meet the resident’s medically related physical, social, and behavioral health needs.” COMAR 10.07.02.30 Presents pertinent information influencing patients/residents’ response to disease process or treatment goals and prevention of reoccurring illness/injury. Provides therapeutic counseling to patients/residents as part of their treatment plan to identify barriers to achieve the highest level of independence. Maintains written records and documentation in accordance with Federal and State regulations and in accordance with facility policy.

Collaborates with the patient/resident, family, guardian and/or identified support person, as well as the Interdisciplinary Treatment Team to establish patient/resident centered goals, and interventions. Collaborate with the patient/resident, family, guardian and/or identified support person to reduce barriers and to maximize the patient/residents’ functional independence. 

Assists Interdisciplinary Treatment Team in identification and review of patient strengths and areas needing support through a culturally competent lens. Assumes leadership in the development of patient/resident centered care plan goals and to monitor progress.  

Reviews care plan goals with the patient/family, as required. 

Provides patient/family education. 

Coordinates discharge planning with the patient/residents’, family, guardian, and/or other identified support persons, community agencies, stakeholders, and the Interdisciplinary Team. Complete a discharge assessment of needs and develop a plan to best meet patient/residents' need, and follow-up after discharge, as needed.

Facilitate supportive relationships through WMHC and with community support agencies and stakeholders. 

The treatment team is educated on these components of care and how they will affect achievement of the treatment goals.

Maintains written records on all patients: biopsychosocial-spiritual assessment; progress notes; care plans; discharge summaries. Support in obtaining patient/resident advanced directives, guardianship order, and update insurance information, as needed, and as directed by policies and/or management.

Expands knowledge base of community resources and improves professional skills through attendance at educational programs and professional meetings.

Attends mandatory education and maintains all license requirements. 

Keeps abreast of changes in Medicare/Medicaid coverage and requirements.    

MINIMUM QUALIFICATIONS

Education:   Determined by the Maryland State Board of Social Work Examiners under the licensing requirements for Social Workers.

Experience: Two years of experience rendering clinical social work services in a health care or treatment setting subsequent to the receipt of an approved Master’s degree in Social Work from an accredited college or university.

Note: Candidates may substitute U.S. Armed Forces military service experience at a rank of Corporal/Petty Officer or higher as a commissioned officer in Social Work classifications or Social Work specialty codes in the Social Science, Psychology, and Welfare field of work on a year-for-year basis for the required experience.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

1. Candidates must be licensed as a Certified Social Worker or Certified Social Worker, Clinical by the Maryland State Board of Social Work Examiners prior to appointment to a position in State service. Applicants who do not have a license may also apply pending receipt of the required license. When the license is obtained, submit a copy to the Maryland Department of Health Testing Unit.

2. Candidates appointed to positions in this classification may be assigned duties which require the operation of an automobile. Employees assigned such duties will be required to possess an appropriate operator's license valid in the State of Maryland.
