Main Purpose of Job

The Community Clinical Linkages Team, which resides in the Center for Chronic Disease Prevention and Control (CCDPC), is responsible for a dynamic portfolio of projects that facilitate healthy lifestyles for Marylanders -- with a focus on diabetes, heart disease, and stroke prevention. The Community Programs Coordinator plans, implements and evaluates activities related to statewide, specialized community-based programs, such as worksite initiatives and faith-based engagement. The incumbent will coordinate efforts with other programs within the CCDPC and serves as a health education consultant to local health departments and community-based prevention and health promotion agencies. The Community Programs Coordinator promotes evidence-based practices to improve population health and is responsible for sustainable community-based chronic disease prevention programming across the state, requiring representation and coordination with key stakeholders in both the public and private sectors.

MINIMUM QUALIFICATIONS

Education: Possession of a Bachelor’s degree from an accredited college or university in Public Health Education, Community Health Education or Health Science.

Experience: Two years of experience planning, developing, implementing and promoting health education projects.

Notes: 

1. Possession of a Bachelor’s degree from an accredited college or university with a minimum of 18 credit hours in Public Health Education, Community Health Education, Health Science or related behavioral science and three years of experience planning, developing, implementing and promoting health education projects may be substituted for the required education and experience.  A copy of your transcripts (official or unofficial) must be submitted with your application if using this substitution.

2. A Master’s degree in Public Health, Community Health Education, or Health Science may be substituted for the required education and one year of the required experience.

3. A Doctorate in Public Health, Community Health Education, Health Science or Public Health Policy may be substituted for the required education and experience.

4. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Educator and Community Health classifications or Health Educator and Community Health specialty codes in the Public Health field of work on a year-for-year basis for the required experience and education.

SELECTIVE QUALIFICATIONS

Candidates must have at least two years of program management experience with a Bachelor's degree or one year of program management experience if applying the Master's degree substitution.  A Doctorate degree in the specified fields will substitute for the experience.

DESIRED OR PREFERRED QUALIFICATIONS

The preferred candidate will have public health programming experience in the field of chronic disease prevention and control. The preferred candidate will also have a Master's degree in public health or a related field.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.

BARGAINING UNIT STATUS

The incumbent in this position is a member of a covered bargaining unit and will be required to pay a bi-weekly service fee to the exclusive representative of the bargaining unit.
