Main Purpose of Job

The Local Health Department (LHD) Community Coordination Manager is a senior management position within the Office of Population Health Improvement (OPHI). This position requires project management expertise.  This position requires an understanding of fiscal and budget management principles. This role manages LHD and other external engagement and coordination functions to assure alignment of all OPHI external engagement processes with broad MDH priorities and the LHD core funding process.  

This position works closely with the OPHI Director, State Health Improvement Process (SHIP) team, fiscal officer and OPHI senior program directors to develop and implement effective and efficient local health department and stakeholder program engagement policy and processes related to population health outcomes, communication, local health department funding and partnerships, and data access and utilization processes. This position will be responsible to align population health activities, stakeholder engagement processes, LHD Core funding and Local Health Improvement Coalition (LHIC) activities (including managing Core funding administration, reporting, and monitoring processes) to align with the SHIP implementation processes. An important aspect of the position is collaboration across MHD Public Health Services (PHS) regarding data processes and activities to support the success of PHS programs and other MDH administrations. This position will serve as the liaison between the MDH programs and the LHICs,  

This position manages the SHIP team (data analyst and two interns working on SHIP stakeholder engagement activities) to assure alignment of OPHI external engagement processes with MDH population health data access and utilization and reporting priorities. This position leads the monitoring and reporting functions related to the Local Health Improvement Coalitions; engages LHICs, Hospitals, to leverage partnerships to address population health measure (s) and align with the TCOC implementation activities.  This position also serves to sustain the LHIC collaboration and coordination activities of the SHIP, coordination of LHICs and other MDH population health data activities.  

This position will be responsible for establishing and overseeing the LHIC monitoring and reporting process; developing, deploying and integrating collaboration efforts between the programs across local partners (e.g., LHD, LHIC, payers, constituents);  developing, deploying and integrating cross-agency collaborative and communication processes related LHD engagement in population health activities within OPHI and MDH more broadly through  enhanced communication with the local health departments and local health improvement coalitions; and engaging the SHIP team to enhance external stakeholder communication processes such as the SHIP newsletter, or convenings and TA to LHICs, to maximize local engagement that reflect and respond to local needs.

MINIMUM QUALIFICATIONS

Bachelor’s degree and six years of health or human services administration or professional experience, including three years at a supervisory or management level.  A Master’s degree will substitute for one year of general experience, a Doctorate for two years of general experience.  Additional experience will substitute year-for-year for the college degree.
