Main Purpose of Job

The main purpose of this position is to preserve, promote and protect the health of Anne Arundel County residents by assuring social, medical, and psychological needs of people living with HIV are met while maintaining their independence. The case manager needs to be knowledgeable about all resources available to the HIV positive client and how to access the resources. Under the direct supervision of the Community Health Nurse Supervisor for the HIV/STI Prevention and Care Program, duties include client intake and assessment interviews, developing plan of care, implementation and coordination of plan of care, recommends and/or develops resources to overcome gaps/barriers to client service delivery, educates the client, the client's family or other care providers on these resources and other HIV related issues, facilitates monthly support groups, as well as participation in bi monthly HIV clinic. The position's duties also require data entry skills and use of a computer. The position requires intermittent travel throughout Anne Arundel County.

MINIMUM QUALIFICATIONS

Education:   Determined by the Maryland State Board of Social Work Examiners under the licensing requirements for Social Workers.

Experience: Two years of experience rendering clinical social work services in a health care or treatment setting subsequent to the receipt of an approved Master’s degree in Social Work from an accredited college or university.

Note: Candidates may substitute U.S. Armed Forces military service experience at a rank of Corporal/Petty Officer or higher as a commissioned officer in Social Work classifications or Social Work specialty codes in the Social Science, Psychology, and Welfare field of work on a year-for-year basis for the required experience.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess:
  • Experience in mental health and addiction counseling preferred.
  • Experience with undeserved populations preferred.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

1. Candidates must be licensed as a Certified Social Worker or Certified Social Worker, Clinical by the Maryland State Board of Social Work Examiners prior to appointment to a position in State service. Applicants who do not have a license may also apply pending receipt of the required license. When the license is obtained, submit a copy to the Maryland Department of Health Testing Unit.

2. Candidates appointed to positions in this classification may be assigned duties which require the operation of an automobile. Employees assigned such duties will be required to possess an appropriate operator's license valid in the State of Maryland.
