MDH Somerset County Health Department, Westover MD

Main Purpose of Job

This position will be responsible for the day to day coordination of services for homeless individuals and/or families with children whose head of household has been diagnosed with a serious mental illness. The individuals and/or families are placed in supportive housing and receive case management who will play an active role with housing placement. The Case Manager will provide linkage to community services such as education services, job training and placement. This position advocates on behalf of the participants working directly with them to identify the specific challenges they may be facing to ensure that the program participants maintain permanent housing and become self-sufficient.

MINIMUM QUALIFICATIONS

Education: Possession of a bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

Notes: 1. A bachelor’s degree from an accredited college or university in another field plus one year of professional experience in health services may be substituted for the specific degree.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess experience working in a behavioral health setting and experience with data entry/computer skills and customer service.
