Main Purpose of Job

Plans for, develops and administers programs and intervention that achieve and sustain health and wellness outcomes for children and adults. Coordinates holistic healthcare services for vulnerable children and adults by partnering with child welfare and adult services caseworkers, biological parents, resource home providers, managed care organizations, and healthcare providers. Ensures the health care needs of children in foster care are being met in a timely and effective manner and are in compliance with state and federal mandates. Identifies children at risk for health issues/exacerbation of illness and monitors closely. Provides resources, recommendations, and education to biological and adult and child resource parents caregivers to maintain health and wellness. Completes home visits as necessary to assess children' and adults health needs and provides education to caregivers. Assists resource families with navigating managed care systems in establishing a medical home for adults and children in the SMCDSS social services division. Prioritizes care and contacts based on urgency-including the ability to multi-task with accuracy. Documents case management assessments, activity, treatment plans and resolutions in CJAMS. Participates in Family Team Decision Making Meetings as required, in addition to staff, community, professional and inter-agency meetings and conferences. Provides education to clients and SMCDSS staff. May provide training on health-related topics to Project Home and Resource Parents.

MINIMUM QUALIFICATIONS

Education:   A Bachelor’s degree in nursing from an accredited college or university.

Experience: None.

Notes:

1. Candidates may substitute a Bachelor’s degree in a related field from an accredited college or university and one year of experience as a Registered Nurse for the required education.

2. Candidates may substitute two years of experience as a Registered Nurse for the required education.

3. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Nursing classification or Nursing or Health Care Specialist specialty codes in the Nursing field of work on a year-for-year basis for the required education.  

DESIRED OR PREFERRED QUALIFICATIONS

Preference will be given to applicants who possess the following preferred qualification(s). Include clear and specific information on your application regarding your qualifications.

One year experience using computer software such as Google Applications and Microsoft Suites. 

One year experience using Case Management software. 

One year experience working for a local or state government agency. 

One year experience leading a team or program.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

1. Candidates for positions in this classification must possess a current license as a Registered Nurse from the Maryland State Board of Nursing, 4140 Patterson Avenue, Baltimore, Maryland 21215.

2. Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.

SPECIAL REQUIREMENTS
