Main Purpose of Job

The Maryland Primary Care Program (MDPCP) is an essential part of the overall transformation of health care delivery in Maryland. In order to successfully implement this change, the Program Management Office is hiring a team of individuals to provide hands-on technical assistance and support to practices in three major areas: (1) quality and data components of the program; (2) practice transformation strategies to meet the care transformation requirements as defined by the MDPCP implementation guide. (3) implementation of COVID-19 protocols including vaccination, testing, and treatment.

Coaches will be expected to connect and work with practices in-person and electronically, develop and use tools to engage practices (e.g. toolkit), and serve as an account manager for participating practices. Coaches will be available on request to all of their assigned practices and Care Transformation Organizations (CTOs). Practice coaches will also work closely with the MDPCP Learning System and support initiatives related to the COVID-19 Public Health Emergency to support primary care practices in caring for their patients.


The position requires a bachelor’s degree from an accredited University and the ability to travel (owned vehicle). Individual should have their own cell phone for work in the field.

The ideal candidate will have 5+ years’ experience in an external, customer-facing position in health care. Candidate should be mission-driven. Candidate should have some health care background in directly working with community providers in a consulting or employee relationship providing expertise on provider communications, networks, communications, legislation, policy, data, HIT or other related topics. Candidate should be highly knowledgeable about health care transformation, value-based payment, primary care, health and hospital systems, and trends in the delivery of health care, including how it applies to Maryland primary care providers. Candidate should be familiar with the landscape of providers in Maryland including primary care practices, hospitals, and Accountable Care Organizations.

Candidates may remain on the certified eligible list for a period of at least one year. The resulting certified eligible list for this recruitment may be used for similar positions in this or other State agencies.


Contractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program, have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits.

Leave may be granted to a contractual employee who has worked 120 days in a 12 month period. This leave accrues at a rate of one hour for every 30 hours worked, not to exceed 40 hours per calendar year.
