POSITION DUTIES

The main role of this position as the Resource Linkage and Care Coordination Services Grant Monitor is to provide program oversight, track funding, offer technical assistance, report completion for the Projects for Assistance in Transition from Homelessness -PATH, Mobile Crisis Team-MCT (State and County), Mobile Crisis Hotline-MCH, and Behavioral Health Navigation (BHN) Grants. The position will serve as the point of contact for the SSI/SSDI Outreach, Assistance and Referrals- SOAR, and ensure that implementation of all services are in accordance with the Conditions of Award funding. This position will also assist in conducting, documenting and tracking Residential Rehabilitation Program placements and inspections for Howard County referrals as well as address any complaint/critical incidents, investigations/resolutions pertaining to monitored grants. This position will also conduct bi-annual site visits and maintain appropriate documentation of these visits.

This position is eligible for hybrid telework.

80% Grant Monitoring of Contracts/Tracking of Funding

  • Grant monitoring of the Projects for Assistance in Transition from Homelessness PATH, Mobile Crisis Team-MCT (State and County), Mobile Crisis Hotline-MCH, and Behavioral Health Navigation (BHN) Grants. The position will serve as the point of contact for the SSI/SSDI Outreach, Assistance and Referrals- SOAR, and ensure that implementation of all services are in accordance with the Conditions of Award funding to ensure in accordance with the Behavioral Health Administration’s Conditions of Award/Scope of Work. 

  • This position will also assist in conducting, documenting and tracking Residential Rehabilitation Program placements and inspections for Howard County referrals 

  • Submit program relevant data and updates monthly/quarterly/bi-annually to the Behavioral Health Administration (BHA)/Bureau Director by due dates. 

  • Address contract vendor questions, and is the point of contact for the vendors, and provide technical assistance.  

  • Monitor contract to ensure in compliance with performance standards, expense limits, and service provisions. Conduct grant monitoring/site visits bi-annually and submit audit documentation to the Behavioral Health Administration (BHA)/Bureau Director by due dates.  

  • Review/approve invoices associated with contracts in preparation for payment. Works with Fiscal Services/Contracts Unit to address any concerns with invoices, contracts, or purchase orders (if applicable). Submit invoices to the Bureau Director for payment approval in a timely manner after ensuring the contract and purchase order (if applicable) have been executed and funding is sufficient.    

  • Track contract expiration and renewal dates to mitigate gaps in services.  Work with the Contracts Unit to ensure contract creations, renewals and purchase orders are completed by due dates. Inform the Bureau Director of any contract creation/renewal/purchase order concerns, as soon as possible. 

  • Draft new contract worksheets with new vendors. Work with the Contracts Unit to ensure contracts are drafted within specification.  Gain final contract approval from Bureau Director. 

  • Lead efforts working with the vendor to ensure 432 packets and 440s are submitted on time and adequately completed. 

20% Behavioral Health Resource Linkage/ Care Coordination

  • Investigate complaints/critical incidents.   

  • Respond to complaints/critical incidents within five (5) business days.   

  • Work with the Behavioral Health Administration (BHA)/Bureau Director to resolve complaints.   

  • Document complaints/critical incident and submits report to the Behavioral Health Administration (BHA)/Bureau Director by due date. 

  • Conducts audits with Administrative System Organization (ASO) and BHA  

  • Review Performance Improvement Plans (PIP) completed by behavioral health providers. 

Performs other related duties, including but not limited to: 
  • Accept responsibility for special projects as assigned.  
  • Participate in appropriate professional education and training for roles and responsibilities of work assignments, as requested or approved by Supervisor. 
  • Participate in Emergency Preparedness drills or actual events.   

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: Two years of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university on a year-for-year basis for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

Desired Qualifications:
  • Experience with grant monitoring, which includes but is not limited to the tracking of funding, planning and implementation of the conditions of award.
  • Organized and resourceful with solutions to address partner agency inquiries and requests for technical assistance.
  • Experience with collecting and analyzing data that will inform program planning and adjustments.
  • Ability to guide vendors through the start-up and close-out budgeting, contractual and reporting processes within a fiscal year.
  • Ability to conduct presentations with community and behavioral health providers as needed on services provided.
