Main Purpose of Job

This position will act as regional lead for Self-Directed Services (40%). This includes attending meetings with other statewide SDS representatives, providing training and technical assistance to CCS, providers/vendors, and families. This position will assist in the development of policy and procedures to ensure consistency and compliance with all state and federal standards. This position will review requests for Individual and Family Directed Goods and Services to ensure the request meets program requirements for approval. This position will complete correspondence if IFDGS request is denied so all appropriate appeal rights are provided. This position will complete IFDGS reviews in appropriate timeframes. This position will also manage the day-to-day operations of the Eligibility and Access process (60%) within the state regulations framework that is designed to assess the eligibility status for individuals with developmental disabilities who are applying for services funded by the DDA in the Eastern Region. The Eligibility and Access position’s principal responsibilities are to process eligibility and access applications, send appropriate letters, enter information into LTSS and PCIS2. This position will manage CCS assignments for the Comprehensive Assessment, waitlist monitoring, and regional transfers. This position will maintain the DDA Eligibility tracker, and maintain Choice Letter assignments along with all eligibility documents in the ESRO digital filing system, and must adhere to strict timeframes for statewide compliance. This position will also review and determine the eligibility for individuals applying for DDA services and provide technical assistance to the CCS's who facilitate the eligibility and priority process. This job is eligible for telework subject to the approval of the Regional Director.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: Two years of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university on a year-for-year basis for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The preferred candidate should have direct experience working with ID/DD, and case management experience.
