POSITION DUTIES

The School-Based Wellness Center (SBWC) Case Manager provides coordination services to students and their families enrolled in a School-Based Wellness Center who require additional services outside of the SBWC. The Case Manager will review provider recommendations, student/parent needs/requests and provide information on applicable resources, facilitate referrals to care including all follow up needs, community support programs and services, and other social determinants of health programs for students and families. This position will create and maintain a local resource directory for providers who have uninsured specialty care. Will work with the HCHD ACCU and CareApp to get students insured when possible and refer to community resources as needed.

Day shift with evenings and weekends as needed for special events. Eligible for hybrid telework schedule.

60% Facilitate linkages to care and services recommended by the SBWC Provider.  Receive information from SBWC provider and contact families to determine needs and recommend applicable resources.  Facilitate warm hand off to points-of-contact at referred resources. Determine barriers to care and create plan to alleviate barriers.  Develop and maintain resources For children and families with health challenges.

15% Assist with program administrative duties and special projects as assigned. Work with the SBWC staff to ensure day to day operations of the SBWC program. 

10% Conduct outreach with schools and families including attending school activities, association meetings, and clinic events.  Provide community education on SBWC and health department programs as requested, 

10% Act as Liaison with Outside Providers – develops professional relationships with clinical and non-clinical support providers; works to alleviate burden on students/families and providers by ensuring warm hand offs and assisting with follow up needs; serves as a liaison for outside providers to connect students/families to community resources.

5% Other duties as assigned – Participates in professional education and training for roles and responsibilities of work assignments; Participates in emergency preparedness drills or actual events.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:
1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

Desired qualifications:
  • Experience with collaborating effectively with individuals from diverse backgrounds, demonstrating professionalism and maintaining composure in potentially stressful situations
  • Bilingual in Spanish
  • Experience with working independently with minimal supervision, demonstrating strong self-motivation and effective time management skills
  • Experience with case management (e.g. ability to follow up on appointments and referrals)
  • Excellent organizational skills, with a proven ability to manage multiple tasks and projects efficiently
  • Excellent verbal and written communication skills
