Main Purpose of Job

The Center for Chronic Disease Prevention and Control (CCDPC) is responsible for directing the dynamic team for a broad portfolio of projects that facilitate healthy lifestyles for Marylanders, with a focus on diabetes, heart disease, and stroke prevention, disabilities inclusion, and cognitive health programming. The Chronic Disease Program Manager will oversee and coordinate operations of multiple grants, lead the referral efforts of the CCDPC, and oversee at least one federal grant. The Chronic Disease Program Manager will actively seek grant opportunities, and direct grant writing teams for grant submissions. The Chronic Disease Program Manager will have expertise in grant writing and management, as well as procurement, and budget development and oversight. The Chronic Disease Program Manager will participate in state efforts to secure referral systems for evidence-based programs, and lead CCDPC referral efforts. The Chronic Disease Program Manager will evaluate efficiency within the grant and procurement processes in CCDPC and will make recommendations to the Director on new policies, standards, and workflow procedures. The incumbent will assure staff properly manage contracts with external partners.

MINIMUM QUALIFICATIONS

Qualified candidates must possess a bachelor's degree from an accredited college or university and 6 year of professional health service experience, including 3 years at a supervisory or managerial level.  Candidates must also possess at least 2 years of experience with public health programs, including current health care delivery systems, barriers to access, and social detriments of health.

Additional professional health services experience can be substituted for the education requirement at the rate of 1 years of experience for 30 credits hours, up to 4 years.  A master's degree will substitute for 2 years of professional health services experience.
