Purpose

The Scales Neighborhood Nursing (SNN) Program Manager leads the delivery of public health services to improve community health outcomes in designated neighborhoods.

Essential Functions

Program Management

  • Monitor and evaluate programmatic progress, making data-driven decisions to adapt strategies and services.
  • Foster collaboration and communication within the team, across departments, and with external partners to maximize the program’s impact.
  • Ensure that activities align with organizational goals, public health standards, and community needs.
  • Manage policies and resources, including staffing, budget, and materials, to ensure efficient and sustainable operations.
  • Oversee staff performance, providing mentorship, training, and feedback to support professional growth and enhance service quality.
  • Ensure compliance with regulatory requirements and maintain accurate documentation for programmatic evaluation and reporting.
  • Review and analyze national, regional, state, and local data to identify trends in public health, substance use, mental health, and the social determinants of health; recommend prevention and intervention strategies.

Community Partnership, Collaboration, and Leadership

  • Develop and maintain professional relationships with healthcare provider organizations, businesses, other governmental agencies, and community-based organizations (CBOs).
  • Prepare reports, briefings, and updates regarding interactions with outside agencies.
  • Facilitate clinical learning experiences and site visits for program partners and grantors.
  • Provide professional expertise to research project proposals and undertakings.
  • Partner with MHD programs to integrate priorities within the community, including communicable disease and immunization services.
  • Provide leadership in assigned workgroups, committees, or task forces, including those related to public health emergency preparedness and response.

Information and Data Management

  • Oversee the collection, organization, analysis, and reporting of client data to support service delivery and program evaluation.
  • Ensure that all data is accurate, consistent, and compliant with local, state, and federal regulations, particularly concerning patient confidentiality.
  • Implement and maintain data systems that track program outcomes and key performance indicators, and provide performance and outcome data to support informed decision-making and continuous quality improvement.

Compliance and Safety

  • Ensure that operations align with public health standards.
  • Maintain current knowledge of professional licensing and scope of practice requirements for clinical staff, ensuring that staff maintain required licenses and credentials.
  • Ensure the SNN Program adheres to all applicable health and safety regulations, workplace safety protocols, and legal compliance standards.
  • Provide leadership and follow-up related to incidents, errors, and other patient service issues; oversee action related to safety and emergency preparedness.
  • Support the implementation of policies, protocols, and process documentation, in collaboration with the medical director or physician consultant.

Conditions of Employment

  • Work schedule may include weekends and evenings, as required to meet the needs of the position.

  • Pre-Employment Vaccination and Screening Requirements:
    • Annual TB Test
    • Annual Covid-19 and Influenza vaccinations
    • Pre-employment immunity verification required for MMR, Varicella, and Hepatitis B
      • If not immune, vaccination or an approved exemption is required.
      • Vaccination series only needs to be done once.

Minimum Requirements

  1. Bachelor’s degree in public administration, public policy, human services, nursing, public health, or a related field from an accredited college or university.
  2. Two years of progressively responsible experience in public health care program planning, policy development, community health assessment, or health administration, including one year supervising staff and providing program management.
  3. Valid driver's license and possession of a properly insured vehicle for use on the job are required at the time of appointment and throughout employment. Private auto allowance may be paid pursuant to Section 350-183 of the Milwaukee Code.

Equivalent combinations of education and experience may also be considered, however one year of supervising staff and providing program management is required and cannot be substituted.

Technical

  • Knowledge of public health and the ability to translate knowledge into program objectives.
  • Knowledge of best practices for managing public or community health programs.
  • Knowledge of related community resources and outreach principles and practices.
  • Ability to stay up to date on public health issues and best practices that impact the local community.
  • Knowledge of and ability to effectively utilize proprietary software, including OCHIN Epic Electronic Health Records, the Wisconsin Electronic Disease Surveillance System (WEDDS), and the Wisconsin Immunization Registry (WIR).
  • Knowledge and ability to work with computer applications, including the internet, spreadsheets, databases, and word processing software.
  • Ability to read, analyze, and interpret professional journals, technical procedures, and regulations.
  • Skill in analyzing and evaluating data and information and making appropriate recommendations.
  • Ability to track and monitor data and reports.
  • Ability to assist in the creation and management of programmatic budgets, grant proposals, and contracts.

Supervision

  • Knowledge of supervisory principles and concepts, including administrative and reflective supervision.
  • Ability to effectively plan and delegate work, manage multiple priorities, and work within tight time constraints to meet deadlines.
  • Ability to assign duties, set performance standards, provide guidance and training, monitor work in progress, evaluate performance, and make hiring recommendations.
  • Ability to assist in the development, integration, and monitoring of program goals, objectives, priorities, and outcomes.
  • Ability to lead, manage, and motivate a diverse team.
  • Ability to identify and improve workplace processes.

  • Written communication skills, including the ability to write narrative case notes, routine reports, professional correspondence, and procedure manuals.
  • Verbal communication skills, including the ability to speak clearly and persuasively, effectively present information, and respond to questions from managers, clients, and the public.
  • Ability to develop and maintain positive working relationships with a diverse clientele, multi-cultural and multi-disciplinary staff, community organizations, City officials, other agencies, and the public.

Current Salary

The current salary range (Pay Range 1GX) is $85,086-$101,664 annually, and the resident incentive salary range for City of Milwaukee residents is $88,490-$105,731 annually. Appointment above the minimum is possible based upon level of experience and other qualifications and is subject to approval.

Selection Process

THE SELECTION PROCESS will be job-related and will consist of one or more of the following: an evaluation of education, experience, and/or responses to supplemental questions, a written or performance test, a structured interview, or other assessment methods. The Department of Employee Relations reserves the right to call only the most qualified candidates to structured interviews and performance examinations. Structured interviews may include written exercises. Selection process component weights will be determined by further analysis of the job.
