Purpose
Empowering Families of Milwaukee (EFM) is a long-term home visitation program that serves pregnant women and families with young children. The program applies an evidence-based model to improve outcomes for Milwaukee's families. The EFM program staff collaborate with community partners to facilitate access to health, social and child development support.
The Public Health Nurse 1 in the EFM program conducts personal/home visits to work intensively with at-risk pregnant women, new mothers, infants, and young children to improve maternal health, prenatal care, and early childhood health, development, and well-being.
The nursing assignments for this recruitment will be in the Empowering Families
of Milwaukee (EFM) program.
Essential Functions
Home Visiting and Case Management
- Develop and maintain supportive and trusting relationships with clients and other family members to promote independent and empowered actions by families.
- Provide on-going home visits and phone contact, based on the family’s needs as prescribed by EFM protocols.
- Utilize the evidence-based Parents as Teachers home visitation model.
- Assess client/family needs to make referrals to community resources.
- Act as a liaison and an advocate for families regarding needed resources from governmental, community and private agencies.
- Provide public health nursing case management for families, as prescribed by EFM protocols, to address primary, secondary and tertiary health problems.
- Complete developmental screening and assessment of clients’ physical, social, psychological, and environmental health status.
- Teach health promotion and disease prevention, encouraging adoption of healthy behaviors.
- Develop and implement case management and care plans for the resolution of assessed client/family needs.
- Document nursing case management and home visitation activities within designated MHD/EFM databases and charts, in accordance with EFM documentation protocols and following MHD policy and Medicaid requirements.
Collaboration
- Function as a member of the multidisciplinary team, working in a dyad composed of a Public Health Nurse and Public Health Social Worker.
- Provide case consultation and joint home visits with partners in the Direct Assistance for Dads (DAD) Project, Birth Outcomes Made Better (BOMB) Doula and Parents Nurturing and Caring for their Children (PNCC).
- Collaborate with private care providers, allied professionals and other MHD personnel regarding the needs of clients and coordinate care.
- Conduct community outreach to increase awareness of the EFM and support client recruitment and enrollment into the program.
- Facilitate and conduct client group meetings, events and health education classes.
- Develop networks and relationships with the community and other agencies to serve as a liaison and advocate for clients and populations.
- Represent the MHD in professional organizations and events, supporting MHD and EFM goals, outcomes, policies, and strategies.
- Participate in the coordination and administration of services at health centers/special events.
- Participate in departmental, divisional, and/or program-level committees.
EFM Program Evaluation
- Evaluate and/or provide assistance to determine the effectiveness of particular nursing services and home visiting interventions.
- Participate in data collection, program evaluation, continuous quality improvement, and preparation of reports pertinent to the objectives of the program.
- Assess community needs for input into MHD programs.
Professional Development
- Attend program meetings, training sessions and community meetings.
- Participate in orientation and ongoing training for EFM and MHD.
- Engage in professional development opportunities to enhance and maintain knowledge, skills and abilities in maternal-child health, nursing, home visitation and public health.
Clinical Nursing Services
- Perform MHD public health nursing duties to support the needs of the department and the community.
- Administer immunizations based on MHD policy to specified populations.
- Provide pregnancy testing and appropriate health teaching, information, referrals, and resources.
- Answer information calls and respond to impromptu interactions with individuals from the community, providing appropriate health teaching, information, referrals, and resources.

Conditions of Employment
- Participate in a work schedule that includes occasional weekend and evening hours. The current hours of work are as follows: Monday through Friday, from 8:00 a.m. to 4:45 p.m.
- Must wear industry-related personal protective equipment (PPE) such as gloves, facemask, or face shield.
- May travel outside the City of Milwaukee, occasionally overnight, to attend training and conferences.
- May be required to carry a mobile device during business hours.
- Must be able to lift and move up to 25 lbs.
- May be exposed to infectious agents or toxins.
- May be required to respond to an emergency and/or broad impact event.
- The required pre-employment tests include:
- Physical/lift test
- Audiogram
- Vision
- Medical evaluation for respirator
- Respirator fit test
- Non-DOT drug test
- The required pre-employment vaccines include:
- Annual TB Test
- Annual Covid-19 and Influenza vaccinations
- Pre-employment immunity verification required for MMR,
Varicella, and Hepatitis B
- If not immune, vaccination or an approved exemption is required.
- Vaccination series only needs to be done once.
Minimum Requirements
- Bachelor of Science Degree in Nursing (BSN) from an
accredited nursing program.
(NOTE: Students in their final semester in school may participate in the selection process, but may not be hired until the degree has been completed.) - Current permit or license to practice professional nursing in Wisconsin OR eligibility to receive such permit or license. License must be obtained by the time of appointment and maintained throughout employment.
- Valid driver's license and possession of a properly insured vehicle for use on the job are required at the time of appointment and throughout employment. Private auto allowance may be paid pursuant to Section 350-183 of the Milwaukee Code.

Desirable Qualifications
- Ability to provide interpretation for clients who speak Spanish, Hmong, Burmese, Karen, Rohingya, Swahili, or another language.
- Experience in maternal-child health, home visitation, or public/community health nursing.

- Knowledge of nursing practice and patient health assessment related to community and public health care.
- Knowledge of contemporary theories of child development and parenting.
- Knowledge of Milwaukee’s health care delivery systems, particularly pertaining to maternal and child populations.
- Knowledge of social determinants of health, health equity and racial disparities, specifically in relation to populations in Milwaukee.
- Knowledge of the information and techniques needed to diagnose and treat injuries and diseases.
- Ability to efficiently use database, spreadsheet, word processing, presentation, and case management information systems.
- Knowledge of mathematics to accurately calculate dosages and take various measurements.
- Ability to read and interpret documents such as safety rules and technical materials.

Current Salary
The current salary range (Pay Range 2GN) is $68,146-$78,768 annually, and the resident incentive salary range for City of Milwaukee residents is $70,872-$81,919 annually. Appointment above the minimum is possible based upon level of experience and other qualifications and is subject to approval.

Selection Process
THE SELECTION PROCESS will be job-related and will
consist of one or more of the following: an evaluation of education,
experience, and/or responses to supplemental questions; a written or
performance test, a structured interview, or other assessment methods. The
Department of Employee Relations reserves the right to call only the most
qualified candidates to structured interviews and performance examinations.
Structured interviews may include written exercises. Selection process
component weights will be determined by further analysis of the job.
