The purpose of the Minimum Qualification Supplemental Questionnaire (Questions #1 through #3) is to assess whether the applicant meets the minimum qualifications for the classification. The minimum qualifications have been identified as critical for satisfactory performance in this classification. The information provided must be consistent with the information on your application and is subject to verification. The responses on the Supplemental Questionnaire are mandatory for participation in this recruitment process.

Questions #4 through #11

The purpose of Questions #4 through #11 of this Supplemental Questionnaire is to obtain specific information regarding your training and experience as it relates to the knowledge, skills and abilities for this classification. Candidates who receive a passing score in the Basic Supervisory Examination will have questions #4 through #11 on the Supplemental Questionnaire evaluated and scored. These Training & Experience Evaluation questions (questions #4 through #11) will be worth 40% of the total weight of the selection procedure.
