SUPPLEMENTAL QUESTIONNAIRE INSTRUCTIONS – MINIMUM QUALIFICATIONS

PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY AS THEY CONTAIN IMPORTANT INFORMATION:

The purpose of this Supplemental Questionnaire is to obtain specific information regarding your training and experience as they relate to the knowledge, skills and abilities linked to the duties of the position.

Questions 1-2 will be used to determine whether you possess the education, trainings, and experience necessary to fulfill the minimum qualifications required for this position.

Questions 3-5 will be used to determine whether you possess the experience to determine your rank on the eligible list and will be made available to departmental personnel and management staff to assist in their hiring decisions.

The information you provide on this questionnaire should be consistent with and must be supported by the information on your application and is subject to verification. Please provide detailed responses that are specific to each question. Stating “please see resume” or copying and pasting your resume will result in a deduction of points. Completeness of responses and level of experience will be considered during the rating process. Please provide all information requested even if the information may appear redundant. If you do not include the work experience you are about to describe in the “Work History” section of your application, you will not receive credit for this experience. If you are copying an old application, please take the time to update your work history section before submitting your application.

One year full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40 hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.
