SUPPLEMENTAL QUESTIONNAIRE

PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY AS THEY CONTAIN IMPORTANT INFORMATION

Introduction

The purpose of this Supplemental Questionnaire is to obtain specific information regarding your training and experience as they relate to the knowledge, skills and abilities linked to the duties of the position.

Questions 1-4 will be used to determine whether you possess the education, trainings, and experience necessary to fulfill the minimum qualifications required for this position.

Questions 5-11 will be used to determine whether you possess the experience to determine your rank on the eligible list in conjunction with the Supervisory Test Battery (50%), and will be made available to departmental personnel and management staff to assist in their hiring decisions.

The information you provide on this questionnaire should be consistent with and must be supported by the information on your application, and is subject to verification. Please provide detailed responses that are specific to each question. Stating “please see resume” or copying and pasting your resume will result in a deduction of points. Completeness of responses and level of experience will be considered during the rating process. Please provide all information requested even if the information may appear redundant. If you do not include the work experience you are about to describe in the “Work History” section of your application, you will not receive credit for this experience. Minimum Qualification Supplemental Questionnaire (MQSQ)

Education

Possession of a Bachelor’s Degree from an accredited college or university; AND

Experience

Five (5) years of professional experience in a municipal Public Safety Answering (PSAP) and/or large public safety communications center, with at least two (2) years of which must have been in a lead capacity directing and monitoring the work of others; AND 

Note:  Five (5) years of professional experience must be within the last seven (7) years.

License and Certification:
Possession of or ability to obtain a valid California Class C driver license prior to employment.

SUBSTITUTION:

Education Substitution:  Additional experience as described above may be substituted for the required education on a year to year basis.  One year (2000 hours) of additional qualifying work experience will be considered equivalent to 30 semester units/45 quarter units.
