  1. I certify that my responses are true and complete to the best of my knowledge and that my application may be deemed incomplete and removed from consideration into the San Francisco Fellows Program if I have failed to submit any of the required application materials or information. 
  2. I Acknowledge the City and County of San Francisco does not provide Work Visa Sponsorship.  I certify that I am legally authorized to work in the United States and do not require any type of work authorization that requires sponsorship by the City and County of San Francisco.
  3. As an Applicant to the San Francisco Fellows Program, I will be required to participate in any/all of the following as part of the selection process:
    • Online Assessment for technical competencies
    • Submission and verification of degree(s)
    • Submission and verification of references
    • Phone/Skype Interview(s)
    • In-person interviews held in San Francisco at my own expense
  4. If I am selected as a Fellow, I am available to start work in San Francisco on or by August 10, 2020.  If I am unable to start on or by this date, I acknowledge that I will be removed from the San Francisco Fellows Program.
  5. I understand and agree that any information provided on my application is subject to verification and that any falsification of this application, by intention or omission may result in my disqualification from consideration or dismissal from employment with the City and County of San Francisco
