The purpose of this Supplemental Questionnaire is to identify your job-related experience, training and education in areas that are related to the job of Gardener.  To get started, please answer the following questions below.

INSTRUCTIONS

The following section should ONLY be completed by those applicants who are not currently employed in the City and County of San Francisco's gardener apprenticeship program. If you are currently in this program, do NOT complete the following section. Various tasks are listed that relate to the job of Gardener. Please check the number of years of experience that you have performed each task, if appropriate. After each task, please provide the employer(s) for whom you worked when you performed each task for which you are claiming experience. In addition to the employer's name, you must also indicate the employer's address and a phone number of the employer, a supervisor with that employer or other contact person who can verify that you actually performed the task for the period of time that you indicate on the checklist. Failure to provide this information for a given task will result in loss of credit for that task and may lower your overall score. Consequently, you must complete your questionnaire properly to receive full credit.

Note: Any falsification or misrepresentation of work experience that you report on this questionnaire may not only lead to your disqualification with respect to this job opportunity, but it also may disqualify you for other current or future job opportunities with the City and County of San Francisco.

Task A: Prepares soil by rototilling, grading, spading, and fertilizing to promote plant growth and to prevent drainage problems.

Task B: Maintains plants by planting, transplanting, cultivating, irrigating and pruning to promote plant growth, prevent plant deterioration, and improve aesthetic value.

Task C: Operates and maintains automatic irrigation systems including sprinklers and time clocks sufficient to detect leaks and perform minor repairs as needed.

Task D: Maintains grounds of athletic fields, squares, parks, playgrounds, stadiums, thoroughfares and other landscape areas by clearing brush and debris, sweeping and raking areas, cleaning up litter, clearing drains, and preventing hazardous conditions.

Task E: Maintains lawn areas by raking, edging, mowing, watering, weeding, aerating, fertilizing, over seeding, topdressing and/or other landscaping function(s) to promote lawn health and erosion control.

Task F: Operates power and manual equipment and/or tools such as mowers, water equipment, gopher traps, and other types of gardening equipment and/or tools to assist in maintaining landscape areas.

Task G: Performs routine maintenance on gardening equipment and/or tools to ensure they are maintained and in good working condition.

Task H: Performs general maintenance duties on grounds and facilities such as athletic fields, sandboxes in parks, public easements, restroom facilities, irrigating systems, and other ground and facility areas to ensure clean and safe premises.

Task I: Identify and destroy pests and diseases on plants and/or lawn areas by performing techniques such as Integrated Pest Management (IPM) to control and mitigate the pests and diseases.
