This portion of the Supplemental Questionnaire is to determine if you meet the Minimum Qualifications for Class 7258 Maintenance Machinist Supervisor I position, as well as to determine your knowledge, skills, and abilities in job-related areas that have been identified as critical for satisfactory performance in this position. PART II

This portion of the Supplemental Questionnaire will be scored by an expert panel and will account for 50% of the total weight of the applicant’s final score. Your application or additional attached documents (e.g. resumes, cover letters, letters of reference/recommendation, etc.) will NOT be considered during the scoring process. Insufficient or non-responsive answers to the Supplemental Questionnaire may result in ineligibility, disqualification, or lower scores. It is suggested that you:

- Allow ample time to submit your application and Supplemental Questionnaire responses before the filing deadline.

- Review the questions first, prepare and save your responses in a word processing document, and then paste them into the online Supplemental Questionnaire.

- Be concise but thorough. Ensure that you address all parts of the question. Your written communication skills will be evaluated based on your responses.

Please describe your experience as a Maintenance Machinist.  Include in your response a detailed description of your experience in performing preventive maintenance on motors, pumps, valves, valve operators and large rotating machinery.

Describe your experience with a Computerized Maintenance Management System (CMMS) and its various uses in machine shop.

Maintenance Machinists with the San Francisco Public Utilities Commission (SFPUC), are often required to perform work in confined spaces where minor injuries and/or exposure to various environmental hazards may occur.   Please describe your experience working in confined spaces and how you would safely carry out a confined space entry to ensure the safety of all personnel.
