The Department of Public Health’s San Francisco Health Network (SFHN) is San Francisco’s only complete care system and it includes health care for all ages: primary care, medical and surgical specialties, behavioral health, diagnostic testing, skilled nursing and rehabilitation, emergency and trauma treatment, and dentistry. The SFHN provides primary care in fourteen (14) locations and behavioral health care in sixteen (16) locations throughout San Francisco including, but not limited to clinics focusing on health care for San Francisco’s children, youth, adults, and elders. The SFHN works in partnership with the University of California, San Francisco (UCSF) faculty who provide many services including teaching, research, and patient care at Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG).  

Currently, there are (2) full-time 2233 Supervising Physician Specialist position, functioning in leadership roles for the SFHN primary care clinic/center, program, or other patient care site.  One position will function as the Chief Medical Officer for Primary Care and the other position will function as the Medical Director for the Curry Senior Center.  

  1. The Chief Medical Officer for Primary Care will provide physician leadership and oversight of medical care provided in the SFHN primary care health centers. Primary goals for the position are to create a professional environment which fosters excellence and which is safe, supportive, and sustains an engaged and healthy workforce; to work closely with other SFHN and Primary Care leaders to create systems to support the highest quality care in all SFHN Primary Care health centers while ensuring prompt access to care and a positive and healing experience for all patients; and to integrate our primary care services in order to achieve consistency, efficiency, and effectiveness throughout the SFHN.
  2. The Curry Senior Center is a joint project of the San Francisco Department of Public Health (SFDPH) and Curry Senior Services, a private, nonprofit organization. From its key location, it serves the persistent and urgent needs of low income seniors who are aging in place in the Tenderloin and South of Market districts of San Francisco. Curry Senior Center provides services to seniors that promote independent living while maintaining their dignity and self-esteem. Curry Senior Center serves about 1,600 patients per year in the area of Primary care and preventative health care services and the target population is seniors ages 55 years and older.

ESSENTIAL DUTIES:
Under general administrative direction of the SFHN’s Chief Medical Officer for Primary Care, the 2233 Supervising Physician Specialist performs the following essential job functions:

  • Directs and has overall responsibility for the functioning of a primary care clinic, center, program and other patient care activities, including the assignment and supervision of physician specialists, other health professionals, and other staff members.  This may include making reports, responding to audits and developing and managing a budget for a clinic or program, and also requires “on-call” 24 hour responsibilities;
  • Plans, directs and evaluates medical staff development and in-service training activities at the facility, division, or program;
  • Conducts meetings and trainings with relevant department collaborators and representatives;
  • Provides or supervises direct medical care in the appropriate specialty area and/or coordinates care of particularly difficult and complicated cases through consultation with other medical specialists and community resources;
  • Evaluates community needs for medical services and plans accordingly; conducts and is responsible for on-going professional and community educational programs to supplement and enhance other departmental activities;
  • Directs Quality Management activities by establishing and maintaining a Quality Improvement Program which meets system-wide objectives; creates and maintains an effective Quality Improvement Program infrastructure and demonstrates ongoing ability to identify, measure, and improve quality goals; and
  • When appointed as the Chief Medical Officer for Primary Care, supervises health center Medical Directors at nine (9) or more community-based clinics; creates policies and procedures that support overall/system-wide clinic operations and patient care.  

The 2233 Supervising Physician Specialist performs other related duties as assigned/required.  

Minimum Qualifications

  1. Possession of a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California; AND
  2. Possession of valid Board Certification in the appropriate medical specialty area for the assigned facility or division; AND
  3. Three (3) years (equivalent to 6,000 hours) of post-residency experience as a practicing physician in the respective medical specialty area; AND
  4. Possession of valid Drug Enforcement Agency registration with the United States Department of Justice (Special Condition).  

NOTES REGARDING MINMUM QUALIFICATIONS #2 & #3:

  • The 2233 Supervising Physician Specialist – Chief Medical Officer for Primary Care position requires experience and Board Certification in Internal Medicine, Family Medicine, or Pediatrics medical specialty areas.
  • The 2233 Supervising Physician Specialist - Curry Senior Center Medical Director position requires experience and Board Certification in Internal Medicine, Family Medicine, or Geriatric Medicine medical specialty areas.  

One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40 hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.  Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.

The above minimum qualifications reflect special conditions associated with the position(s) to be filled. They may differ from the standard minimum qualifications associated with this classification. 

CONDITIONS OF EMPLOYMENT:
All qualified candidates who have been selected for appointment to positions in billable specialty areas must be an eligible billable provider and where applicable will be required to meet the following criteria:  

  • Be eligible to participate in Medicare, Medicaid and/or other federal health care programs;
  • Possess a National Provider Identifier (NPI);
  • Submit a completed credentialing application and/or required documentation for credentialing; AND
  • Possess a valid third-party billable provider certification (such as Medicare, Medi-Cal and/or private insurance) OR have submitted a completed billable provider application, along with the required documentation, in order to obtain the appropriate billable provider status.  

Where applicable, failure to meet the criteria and/or ‘Conditions of Employment’ within two (2) weeks PRIOR to start work date may result in the delay of appointment and/or cancellation of employment offer.  

Desirable Qualifications:
The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.

According to assignments, desired qualifications vary, but may include:

  • Experience working with certain vulnerable patient populations
  • Depending on the population served, the ability to speak, read and write in a language other than English
  • Experience with the Lean Management System
  • Proficiency with data management software including, but not limited to Microsoft Excel and Tableau

Selection Procedures

Supplemental Questionnaire/Training and Experience Evaluation (Weight – 100%):

Applicants will be prompted to complete a Minimum Qualification Supplemental Questionnaire as part of the online employment application that must be submitted by the final filing deadline.  This Minimum Qualification Supplemental Questionnaire is designed to obtain specific information regarding an applicant's qualifications.  Applicants must identify their education, experience, training, and licensure/certification in relation to the requirements for this position. The Minimum Qualification Supplemental Questionnaire will assist with evaluating if the applicant possesses the required minimum qualifications.  Once submitted, applicant responses cannot be changed.   

The Supplemental Questionnaire/Training and Experience Evaluation is also designed to measure knowledge, skills, and/or abilities in job-related areas which may include, but not be limited to: 

  • Medical knowledge refers to knowledge in the general practice of medicine which is needed to make expert diagnosis, interpretations and recommendations on a consultative basis; supervise professional and all other support personnel engaged in treatment and services; and to prepare, review, and assist with medical diagnosis, records and reports;
  • Knowledge of supervisory, administrative, and budgetary principles and practices refers to the ability to plan, organize and direct a program; coordinate, execute and assist in the development of policies and methods; direct all personnel and staff development, in-service activities; achieve economies and prevent losses through efficient use of staff at all levels, as well as materials, supplies, and specialty equipment, when necessary; and manage a budget for a clinic or program;
  • Knowledge of community needs refers to knowledge of high-risk populations being served; how to access community resources and/or familiarity with community agencies and other resources which are able to provide assistance to those populations; knowledge of cultural factors which may influence provision of medical care; and knowledge to be able to serve members of diverse backgrounds; and
  • Ability to use effective communication techniques refers to the ability to effectively participate with comparably trained and experienced peers to improve clinical treatment and programs; and to participate with a variety of community risk populations for improvement of services.  

Applicants must achieve a passing score on the Supplemental Questionnaire/Training and Experience Evaluation in order to be ranked on the eligible list/score report.  Successful applicants will be placed on the eligible list/score report, in rank order, according to their final score.  

After the eligible list/score report is finalized/adopted:

  • The department may administer additional selection procedures to make a final hiring decision (e.g. interviews, written exercise, etc.). 
  • Applicants meeting the Minimum Qualifications are not guaranteed advancement through all steps of the selection process. 

Certification:
The certification rule for the eligible list resulting from this examination will be Rule of Ten Scores.  Additional selection processes may be conducted by the hiring department prior to making final hiring decisions.

Eligible List/Score Report:
A confidential eligible list of applicant names that have passed the civil service examination process will be created, and used for certification purposes only. An examination score report will be established, so applicants can view the ranks, final scores and number of eligible candidates. Applicant information, including names of applicants on the eligible list, shall not be made public unless required by law. However, an eligible list shall be made available for public inspection, upon request, once the eligible list is exhausted or expired and referrals resolved. The eligible list/score report resulting from this civil service examination process is subject to change after adoption (e.g., as a result of appeals), as directed by the Human Resources Director or the Civil Service Commission.

The duration of the eligible list resulting from this examination process will be six (6) months, and may be extended with the approval of the Human Resources Director.

Upon approval of the Human Resource Director (see Civil Service Rule 111A.26.5), the eligible list/score report resulting from this announcement may be used by other departments that also use this classification or a similar classification. Conviction History

As a selected candidate for a job, you will be fingerprinted, and your fingerprints will be sent to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI). Disaster Service Workers

All City and County of San Francisco employees are designated Disaster Service Workers through state and local law (California Government Code Section 3100-3109). Employment with the City requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker-related training as assigned, and to return to work as ordered in the event of an emergency.

Note on Personal Protective Equipment (PPE):
Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.  

Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a Face Fitting Respirator.  

Medical Examination:
