Appointment Type: Temporary Exempt (Category 18).  In accordance with San Francisco Charter Section 10.104, exempt employees serve at the discretion of the appointing authority. The duration is up to one year.

The qualified applicant pool resulting from this recruitment may be utilized to fill exempt vacancies at various locations within DPH.

The City and County of San Francisco COVID Command Center (CCC) is transitioning to a long-term staffing model, and we need your help to fill critical positions. We are hiring temporary (up to 12 month) positions to continue and standardize the work performed in our initial disaster response. Shift and location are subject to change depending on the needs of the department- may include weekends. 

GENERAL JOB DESCRIPTION: 
Under general supervision, 
the 2588 Health Worker IV, supervises, coordinates, and instructs a staff of paraprofessional workers in a service program of the Public Health Department; participates in organizing and directing in-service training activities for Health Workers; provides administrative supervision for Health Workers and assists in planning for their most effective use in the assigned program; provides community oriented direct and indirect services; and performs related duties as required. The 2588 Health Worker IV performs the following essential job functions:

  • May supervise a specific health or rehabilitation program; plans, directs, and coordinates the educational, recreational, therapeutic and work projects of the program participants as directed by professional-staff;
  • Supervises and trains lower classification of paraprofessional personnel in the skills required for successful performance of their assigned duties; assists in preparation of in-service training materials; provides liaison with Team Leaders regarding function and performance of Health Workers I, II, and III;
  • Meets with representative of the local community and professional and social agencies to inform them of the scope of available treatment programs and to elicit their cooperation and support; provides community education services;
  • Assists in planning and execution of programs and operations, research projects, and reports, as directed; evaluates data and suggests programs in the preventive areas of social and health adjustments;
  • May interview and screen program clients, identifying general client condition; performs crisis intervention activities under professional direction; provides broad social counseling activities and assists in group therapy sessions; and
  • When assigned to a specialized activity program, is responsible for supervising the program on specified wards or units and/or assisting the director of the program.

The 2588 Health Worker IV performs other related duties as assigned/required.  

Working conditions: Some positions may require employees to work directly with individuals within locked jails or psychiatric facilities, the homeless population, and individuals who have or are carriers of infectious/communicable diseases. Some positions may also require exposure to inclement weather conditions and/or heavy lifting. 

Minimum Qualifications

EXPERIENCEThree (3) years of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties: ; Serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education /information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to targeted populations; escorting and transporting clients; providing courier/dispatcher functions; performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.

EXPERIENCE SUBSTITUTION:  Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.

One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40 hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.

Selection Procedures

Minimum Qualifications Supplemental Questionnaire (MQSQ - Qualifying)  

Candidates will be prompted to complete a Supplemental Questionnaire as a part of the online employment application. All applicants must complete the Supplemental Questionnaire and it must be submitted with the application by the final filing deadline. The information provided on the Supplemental Questionnaire must be consistent with the information on the application and is subject to verification. Applicant responses to the Supplemental Questionnaire will be used to assist in determining if applicants meet the minimum qualifications for the position.

Conviction History

As a selected candidate for a job, you will be fingerprinted, and your fingerprints will be sent to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI). The resulting report of your conviction history (if any) will be used to determine whether the nature of your conviction (or arrest, in limited circumstances) conflicts with the specific duties and responsibilities of the job for which you are a selected candidate. If a conflict exists, you will be asked to present any evidence of rehabilitation that may mitigate the conflict, except when federal or state regulations bar employment in specific circumstances, such as:

Disaster Service Workers

All City and County of San Francisco employees are designated Disaster Service Workers through state and local law (California Government Code Section 3100-3109). Employment with the City requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker-related training as assigned, and to return to work as ordered in the event of an emergency.

Note on Personal Protective Equipment (PPE):

Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing. Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a Face Fitting Respirator.

Condition of Employment (Security Clearance):

Security clearance as issued by the Sheriff’s Department is required as a condition of employment for all positions at the Jail Health Services. Eligibles who have successfully participated in the examination process, received passing scores and have been selected for hire will be required to obtain the security clearance prior to appointment. A photocopy of the security clearance must be provided to the supervisor on or before date of hire. Failure to demonstrate/show proof of security clearance may result in a dismissal and/or termination of employment.

Note: Failure to meet these criteria and/or ‘Conditions of Employment’ within two (2) weeks PRIOR to the start work date may result in the delay of appointment and/or cancellation of the employment offer. Once appointed, all qualified candidates/employees will be required to retain these qualifications throughout their length of employment. Failure to demonstrate (show proof) of qualifications may result in the termination of employment.

Medical Examination/Drug Testing:

Prior to appointment, at the Department's expense, applicants may be required to take a tuberculosis (TB) screening test, a medical examination and/or drug test.
