Appointment Type: Temporary Provisional (TPV).Temporary Provisional appointments may not exceed three (3) years. A provisional appointee must participate and be successful in a Civil Service Examination for this classification and be selected through an open competitive process in order to be considered for permanent appointment.

The San Francisco Department of Public Health (DPH) is accepting applications for one (1) temporary provisional position in class 2587 Health Worker 3. The current position is for the Community Justice Court (CJC) Program. This position requires bilingual proficiency in English and Spanish.

Please note that the applicant pool resulting from this recruitment may be used to fill future TPV vacancies at other locations within DPH.

The Community Justice Center (CJC) works with adults in San Francisco who have been referred by the Collaborative Courts of San Francisco. CJC works alongside the Collaborative Courts and the client by providing strengths-based client centered treatment through a restorative justice lens to individuals seeking to address the various issues in their lives that bring them into the criminal justice system. CJC provides linkage to community-based organizations and community partners to help clients build positive support networks for long-term support thus reducing possibility of recidivism.

Position Description:
Under general supervision, in collaboration with licensed staff, the Health Worker 3 will ensure that individuals seeking access to care are appropriately matched to an appropriate level of intensive treatment. The Health Worker 3 will also represent CJC at collaborative court meetings. The essential duties include, but is not limited to the following:

  • Assist in providing substance abuse and recovery counseling for Community Justice Center (CJC) participants, including outreach to clients who are housed in residential treatment programs; outreach to families and/or natural support system of clients;
  • Transport ambulatory clients between their homes and clinics, hospitals or other social agencies as needed;
  • Complete intake and assessment forms and make appropriate referrals;
  • Utilize objective screening and assessment forms throughout client’s course of treatment;
  • Attend court hearings as scheduled and prepare all relevant court documents needed for collaborative court meetings, consult with CJC staff members on status updates for the clients who have a court hearing, and provide update to court on client’s progress;
  • May assist in facilitating pre-treatment or treatment groups in con-junction with professional staff and document progress in client’s chart as needed;
  • May transport clients from County Jail and orient them to CJC policies and procedures;
  • Work closely with licensed staff in treatment planning specific to client needs; Monitor and document treatment progress;
  • Submit appropriate documentation according to State, Federal, County and HIPAA standards and regulations;
  • Develop and maintain confidential client charts/information;
  • Maintain quality assurance, confidentiality, legal and ethical standards; Seek consultation and complete any mandated reports (e.g. CPS, APS, Tarasoff) or other reports as indicated by CJC policy and procedure;
  • Consult and coordinate with Case Managers around client’s needs and participate in regular case conference meetings with CJC team;
  • Assist in the coordination with other criminal justice agencies of the delivery of time sensitive and thorough information relating to individuals participating in the Community Justice Center program;
  • Advocate for the placement of CJC clients into community-based treatment settings, as well as participate in case reviews related to the clients’ compliance and progress in treatment activities;
  • Liaison with Sheriff Department and community partners to promote CJC day to day activities as well support the needs of CJC clients with appropriate services and resources;
  • Function as a member of the CJC Team and effectively represent treatment concerns in conjunction with professional staff; 
  • Recommend plans for CJC to address the needs of vulnerable populations (i.e. dual/multiple diagnosed, homeless, etc.);
  • Attend meetings, training's, conferences, etc. as required; and
  • Follow DPH, Behavioral Health Services (BHS) and CJC policies and procedures.

The Health Worker III performs other related duties as assigned/required.

Minimum Qualifications


1. Two (2) years of verifiable experience within the last five (5) years, working with a culturally diverse population performing a combination of at least two (2) of the following duties: Serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to targeted populations; escorting and transporting clients; providing courier /dispatcher functions; performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure; AND

2.  Bilingual proficiency in English and Spanish.

Only those eligible candidates who pass the bilingual proficiency test will be considered for bilingual positions. Applicants must indicate on the application form the language(s) in which they claim proficiency.

Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience for minimum qualification #1.

One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40 hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.  Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted

The above minimum qualifications reflect special conditions associated with the position(s) to be filled. They may differ from the standard minimum qualifications associated with this classification. 

Desirable Qualifications: The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.

  • Possession of a valid Alcohol & Drug Programs Counselor Certification accredited by the National Commission for Certifying Agencies (NCCA) per the State of California.

  • Ability to obtain and maintain San Francisco Jail Clearance to conduct assessments, interviews, and follow up with CJC clients while in custody. 

  • Two (2) years of experience working in substance use disorder treatment setting.

  • Strong experience working with diverse staff and client populations in clinical settings.

  • Excellent organizational, writing, analytic, and critical thinking skills.

  • Familiarity with the substance abuse and mental health systems of care and criminal justice systems in San Francisco and surrounding counties.

  • Proficiency and speed with MS applications including Word, Excel, and Power Point.

Selection Procedures

Minimum Qualification Supplemental Questionnaire (Weight: Qualifying): Candidates will be prompted to complete a Supplemental Questionnaire as part of the online employment application. This Minimum Qualification Supplemental Questionnaire is designed to obtain specific information regarding an applicant's experience in relation to the Minimum Qualifications for this position. Applicants must also complete the official application. It is essential that applicants provide complete information in identifying their education, experience and training. The Minimum Qualification Supplemental Questionnaire will be used to evaluate if the applicant possesses the required minimum qualifications.

The selection procedure will include evaluation of applications and the supplemental questionnaire in relation to minimum requirements.  Depending on the number of applicants, the Department may establish and implement additional screening mechanisms to comparatively evaluate qualifications of candidates. 

Language Proficiency Exam
Candidates deemed qualified who meet the special language condition must pass a departmentally approved language proficiency exam prior to appointment.  Only those eligible candidates who pass the bilingual proficiency test will be considered for the bilingual position.

Conviction History

As a selected candidate for a job, you will be fingerprinted, and your fingerprints will be sent to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI). The resulting report of your conviction history (if any) will be used to determine whether the nature of your conviction (or arrest, in limited circumstances) conflicts with the specific duties and responsibilities of the job for which you are a selected candidate. If a conflict exists, you will be asked to present any evidence of rehabilitation that may mitigate the conflict, except when federal or state regulations bar employment in specific circumstances, such as:

Disaster Service Workers

    Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.
