This position is located in the County Adult Assistance Program (CAAP) of the Human Services Agency.
San Francisco's Human Services Agency (HSA) is a centralized resource which delivers public assistance, child welfare, and aging and adult services to the citizens of San Francisco. Its mission is to promote well-being and self-sufficiency among individuals, families, and communities in San Francisco. HSA was formed in 2004 with the merger of two previously existing City departments - the Department of Human Services (DHS) and the Department of Disability and Aging Services (DAS). The office of Early Childhood Education (OECE) joined the Agency in 2014.
What We Do
We provide a safety net for individuals and families by offering income support, community-based living supports, and assistance getting food, housing, and health coverage. We offer programs and services that ensure the protection and safety of children, the elderly, and dependent adults. We help people secure employment through training, job search, and child care assistance.
Under direction, the Senior Behavioral Health Clinician, with the management team provide oversight of the Social Security Income (SSI) Case Management program including ongoing review quality control of case management activities to ensure that it meets or exceeds project goals. This position supervises a team of 2916 Social Work Specialist assigned to the SSI Case Management Team and Triage Primary Assessment to assist adult clients with disabilities qualify for County Adult Assistance Programs (CAAP) and SSI.
Essential duties include, but are not limited to:
- Ensures case managers’ client assignments are equivalent and workloads are balanced.
- Reviews appropriate forms/documents to ensure accuracy of the findings; ensure clients referred for case management are seen and assessed within established timelines.
- Coordinates with the Disability Evaluation and Consultation Unit (DECU) Clinicians to ensure clients referred to Case Management receive accurate assessments to determine eligibility for SSI.
- Coordinates with the physician, psychologist, and Licensed Clinical Social Worker (LCSW) to ensure clients, who do not have current primary care or behavioral health providers, complete consultative exams with DECU staff.
- Reviews SSI applications for completeness, accuracy, and on-time submittal including recording notes/data for all cases and/or treatment of clients in COSTS.
- Ensure case management staff develops and maintains strong working relationships with referral sources and other staff that work with clients such as: Practical Assessment Exploration Systems (PAES), CAAP Counseling Service, Triage, State Systematic Improvement Plan (SSIP), Shelter Staff, HSA Staff, Single Room Occupancy (SRO) Housing Program, Staff Services Analyst (SSA),and staff and analysts of DDS.
- Coordinates and oversees outstations of Case Management Staff in CAAP Triage, (PAES), Homeless Shelters, and the DHS SRO Housing Subsidy Program.
- Possession of a valid Licensed Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Sciences (BBS); OR
- Possession of a valid Marriage and Family Therapist (MFT) license issued by the California Board of Behavioral Sciences (BBS); OR
- Possession of a valid Professional Clinical Counselor (LPCC) license issued by the California Board of Behavioral Sciences (BBS)
