DEFINITION

Under general supervision, performs health assessments of clients, performs social casework and counseling services, and acts as a liaison to the medical community for the elderly in the Multipurpose Senior Services Program; and does related or other work as required in accordance with Rule 3, Section 3 of the Civil Service Rules.

CLASS CHARACTERISTICS

This is the first level in the MSSP Health Practitioner series. Incumbents in this class carry caseloads that require a working knowledge of nursing care and clinical assessments, in addition to social work and case management techniques.

TYPICAL DUTIES

  • Performs initial physical exam to determine client's overall health status; assesses client's psychological, physical, social and environmental needs; works with client and case managers to develop a realistic plan.
  • Counsels with client and family to develop support systems and to identify community resources for meeting special needs; identifies and recommends purchase of services not provided by the agency.
  • Performs medically related functions for clients; performs periodic health assessments and reviews clinical finds of clients on service; implements consultation for specialist examinations.
  • Monitors client's progress; coordinates with physicians, other health professionals, and pharmacists; consults with client and family to explain and assist with implementing care plan.
  • Maintains detailed, accurate records of all case activities; prepares reports and correspondence.

MINIMUM QUALIFICATIONS

EITHER I
Education: Possession of a Master Degree in Nursing from an accredited school in nursing. 

Experience: Two years experience providing nursing care which involved conducting physical exams and overall health assessments, including one year in public health, geriatrics preferred. 

OR II
Education: Possession of a Bachelor's Degree in Nursing from an accredited four-year college or university. 

Experience: Three years of experience providing nursing care which involved conducting physical exams and overall health assessments, including two years in public health, geriatrics preferred. 

AND
License: Possession of a valid California driver's license. Possession of a valid license to practice as a Registered Nurse in the State of California. 

Certificates: Possession of California State Public Health Nursing Certificate.

KNOWLEDGE

Principles, theories, and techniques of nursing; medical terminology; diagnosis and treatment of medical conditions common to the geriatric population; basic principles of gerontology and the aging process; community resources for the elderly; basic principles of individual and group dynamics; and interactions, side effects and adverse reactions.

ABILITY

Establish rapport with clients and their families; identify medical conditions; assess and monitor client needs; develop and implement client care plans; understand relevant laws, rules and regulations; communicate effectively, both orally and in writing; maintain effective working relationships.

PHYSICAL/MENTAL REQUIREMENTS

Mobility-Occasional operation of keyboards; frequent standing for long periods, walking, pushing/pulling, bending/squatting; frequent driving may be necessary; Lifting-frequently 30 pounds or less; occasionally 70 + pounds with assistance (assisting and supporting patients); Vision-good corrected overall vision required; frequent reading and close-up work; normal hand-eye coordination; Dexterity-normal dexterity with frequent holding, reaching, grasping, writing and repetitive motion; Hearing/Talking-frequent hearing and talking on the telephone and in person; Emotional/Psychological-occasional emergency situations; frequent exposure to trauma, grief; frequent public contact and decision making; frequent concentration required; Special Conditions-my require occasional overtime, weekend or evening work.
