DEFINITION

Under general direction, assesses clients needs; develops plans and services to meet those needs; and does related or other work as required in accordance with Rule 3, Section 3 of the Civil Service Rules.

CLASS CHARACTERISTICS

The Case Manager provides professional counseling, case management and related services for clients served by the Public Health Division of Health Care Services. Within the scope of their assignments, Case Managers work with a relative degree of independence to develop and implement client programs and serve as a resource to other staff.

TYPICAL DUTIES

  • Explains program services and procedures to potential clients.
  • Conducts assessments and evaluations of needs of new and continuing clients.
  • Coordinates the implementation of all aspects of the case plan; develops plan modifications as client needs change or are resolved.
  • Provides counseling and support for clients and their families; serves as client advocate with other agencies; provides crisis intervention.
  • Acts as liaison and facilitator between patient and health care team members; provides consultation in area of specialization; recommends modifications in case management procedures.
  • Assists with general assessment of community needs; assists in developing programs to meet client and community needs; assists in preparing and administering grant proposals.
  • Represents public health programs to other units and community groups; maintains case records; prepares correspondence and reports.
  • May train student interns and other staff.

MINIMUM QUALIFICATIONS

EITHER
Education: A Master’s degree in Social Work from a recognized school of social work based on a two year program which included supervised field placement.

OR
Education: A Master’s degree in Counseling from an accredited college or university with a specialization in Marriage, Family and Child Counseling based on a two year program with coursework sufficient to meet State of California standards for an M.F.C.C. license and which included supervised field placement as part of the course curriculum.

AND
Experience: One year of responsible post Master’s degree experience providing counseling, case management, and development of assistance programs for individuals with public health needs.

KNOWLEDGE

Basic principles of sociology and psychology; principles of human behavior; general case management principles and practices; social economic, and cultural needs and issues facing groups with in the community being served; community organizations and resources; basic public health issues including preventative medicine and treatment and prevention of communicable, chronic and handicapping diseases; principles and techniques of interviewing and counseling; basic principles of public relations.

ABILITY

Assess client needs and determine appropriate services; develop and implement case management plans; counsel and instruct clients and their families; gather and analyze data and prepare appropriate documentation; express self effectively orally and in writing; develop and assist in administering grant proposals; speak before groups; establish and maintain effective working relationships with a variety of clients, staff, public agencies and officials.
