DEFINITION

Under general supervision determines eligibility for AIDS related programs and makes referrals for services and does other related work as required in accordance with Rule 3, Section 3 of the Civil Service Rules.

CLASS CHARACTERISTICS

The employee in this class assists indigent clients to obtain HIV related services through the sponsorship of the Ryan White Consortium of San Joaquin County. Employees determine program eligibility, process applications, provide logistical assistance and refer to authorized county care management agencies for appropriate services.

TYPICAL DUTIES

  • Interview applicants who have been referred to Ryan White Consortium for funding of HIV related case services; complete and processes intake forms; obtains release of information to coordinate health care.
  • Refers eligible clients to appropriate case management agencies for service in areas of medical, transportation, housing, nutrition and counseling.
  • Provides interim logistical assistance in form of food and transportation vouchers and referral to Ryan White Consortium sponsored housing; initiates application process for general relief as appropriate; monitors funds used for housing expenses.
  • Assists and keeps contact with clients through eligibility process; maintains client record throughout period of care management; assures payments to care providers for client services.
  • Meets with case management staff to participate in quality assurance and continuity of care issues.
  • Maintains records, and may prepare reports of activities and prepare correspondence.

MINIMUM QUALIFICATIONS

Education: Completion of 30 semester units of social science or related course work in an accredited college.

Experience: Two years in client services, entitlement programs, social services or related fields working with economically disadvantaged individuals.

Substitution: An additional 30 units of related course work may be substituted for one year of the required experience.

Special Requirement: Ability to speak fluent Spanish or other languages spoken by local cultural groups may be required if relevant to the needs of the job.

KNOWLEDGE

Basic principles and methods of AIDS prevention and transmissions; basic fundamentals of AIDS continuous of illness and course of disease process; methods of interviewing; basic principles of human behavior; basic public relations; language mechanics; basic community resources.

ABILITY

Express self-effectively verbally and in writing; read and understand technical written material; follow directions; work effectively and non-judgmentally with persons with HIV disease and non-traditional lifestyles; establish and maintain effective working relationship with fellow workers; professional staff and other agencies; prepare and maintain appropriate records, play an active role in coordinating resources to meet the needs of clients.

PHYSICAL/MENTAL REQUIREMENTS

Mobility-Frequent operation of a keyboard and sitting for long periods; occasional standing for long periods, walking, pushing/pulling, bending/squatting, driving and stair climbing; Frequency of Lifting-Occasional lifting of 5 to 30 lbs.; Visual Requirements-Frequent use of overall vision, color perception, hand/eye coordination, depth perception, reading/close up work and field of vision and peripheral vision; Dexterity-Frequent holding, reaching, grasping, repetitive motion and writing; Hearing/Talking-Frequent hearing of normal speech, hearing on the telephone/radio, talking in person and on the telephone; occasional hearing of faint sounds; Emotional/Psychological Factors-Frequent exposure to trauma, grief and death, public contact, decision making and concentration; occasional emergency situations, heights, hazardous materials, working alone or on weekends or nights and overtime or travel; Environmental Conditions-Occasional exposure to noise and hazardous materials.
