Behavioral Health Unit Case Manager

Introduction

This is a professional case manager position within the behavioral health role providing crisis-oriented follow-up services for juveniles and/or adults suffering with mental health and/or substance use disorders.

Nature Of Work

An employee in this class is a member of a behavioral health team consisting of licensed clinicians, sworn officers of the Tampa Police Department (TPD) and supervisors. A significant amount of time will be spent coordinating services for citizens with local service providers, participating in multidisciplinary meetings, reviewing reports, notes, and referrals from the team. This position will focus on individuals identified as high utilizers of crisis stabilization units, emergency rooms, and calls for police service related to people who suffer with mental health and/or substance abuse issues. The employee will recognize and appropriately assist as needed to coordinate services, facilitate engagement, and address barriers to minimize incidents of hospitalization or incarceration; report treatment needs and discuss with interdisciplinary team. Work is reviewed through conferences, oral and written reports, and observation of results obtained.

Examples of Duties

Accepts and receives referrals to facilitate the appropriate connection with needed wrap around services with agency case management/persons of contact and clients.

Identify and recommend the service needs of individuals who have been identified as high utilizers of crisis stabilization units, emergency rooms, and/or calls for service for treatment needs and appropriate behavioral health service providers and programs.

Consults with BHU team to determine appropriate dispositions for services, arrests, diversion from arrests, diversion from unnecessary emergency department visits, or some combination of criminal justice/recovery treatment service.

File ex-parte orders as deemed clinically appropriate.

Ensure successful engagement and linkage to referred resources and/or community outreach programs.

Maintain contact with the care coordinators and community providers to follow up on linkage to services and service outcomes to ensure the provision of comprehensive continuum of care and services to promote wellness and recovery.

Attend multidisciplinary treatment team meetings.

Document all client contacts, care and response, maintain reports and updated rosters; collect data and prepare reports as mandated by the program and procedure within agency standards and timeframes.

Record and report all grant needed data for TPD and other involved entities.

Identify trends, patterns and/or the gaps in the community and the department where case management engagement would be most beneficial.

Assist in the review and development of clinical policies, procedures and guidelines for provision of services to assure compliance with all levels of regulations.

Perform related work as required or assigned.


Knowledge, Skills & Abilities

Working Knowledge of case management organization and customer service methods, principles, and practices related to mental health and substance use treatment. Working knowledge of rapport building, engagement practices, impacts of trauma, and training in the field of mental health & substance abuse. Computer skills such as data entry, data tracking, virtual meetings, power point, excel, word, and office equipment required.

Some Knowledge of statistics preferred.

Ability to: work independently and effectively communicate professionally with various organizations and other law enforcement agencies, speak effectively in public, establish, and maintain effective working relationships with other employees, officials, and the public, maintain records and submit clear, concise, and accurate reports both orally and in writing, follow oral and written instructions and arrive at sound decisions independently following established rules, regulations, and state and federal law; maintain confidentiality with police investigations and/or communications overhead as well as clinical behavioral health information, work 10-hour shifts. 

Physical Requirements

Ability to lift 20-40 lbs., mostly sedentary, indoors, requires extended use of computer, typing and viewing monitor.

Minimum Qualifications

Graduation from an accredited college or university with a bachelor's degree and preferably two years of experience with case management. Bachelor’s degree in social service-related field preferred.

Licenses or Certifications

Possession of a valid Florida driver's license required.

Examination

Evaluation of education and experience. Drug testing is included in all pre-employment processing.

Comments

Employees may be required to work rotating shifts, including nights, weekends, holidays and overtime as needed. During periods when the Mayor issues an emergency declaration for the City of Tampa, all employees may be required to work in preparation, response or recovery activities related to the stated emergency.
