Lead is a poisonous substance found in most homes in New Haven, especially those built before 1978. In fact, over 80% of New Haven homes are likely to contain lead. When exposed to lead, children or adults can have permanent and severe health problems. 

The New Haven Health Department works with residents to take simple steps to lower exposure to lead. Our goal is for the New Haven community to stay safe from lead poisoning.

The Community Health Worker will function as part of a team which reports to a Division Director.

PRIMARY FUNCTIONS

This is a full-time position with the New Haven Health Department as a Community Health Worker operating in the field to conduct outreach and educate people who are at risk for acute and/or chronic diseases of public health interest. This position occasionally requires work to be performed in the evenings and on weekends.

The Community Health Worker will function as part of a team which reports to a Division Director.

The specific assignment is responsible for conducting outreach and education for families of children who are at risk or have been exposed to lead poison. The employee in this role will assist affected clients to develop health management plans and goals which include obtaining medical services and participation in the lead abatement program.

TYPICAL DUTIES AND RESPONSIBILITIES

  • Collaborates with key community institutions, organizations, agencies, and individuals to plan, develop, and pilot innovative disease prevention strategies.
  • Assists clients to develop health management plans and goals.
  • Coaches clients in goal setting and effective self-management of their acute and chronic health conditions.
  • Helps clients access medical and non-medical resources, including medical care, substance abuse treatment programs, housing programs, food services, etc.
  • Provides ongoing follow-up, basic motivational interviewing, and goal setting with clients.
  • Conducts program eligibility assessments and enrollment.
  • Disseminates disease prevention materials and information.
  • Administers group education programs in concert with community agencies, groups, and institutions.
  • Maintains records of activities and enters information into database.
  • Handles inventory of outreach project materials, equipment, and supplies.
  • Conducts program evaluations and prepares programmatic reports for funding agency.
  • Performs related work as required.

EDUCATION, QUALIFICATIONS & EXPERIENCE

Possession of an Associates Degree in Public Health, Social Work, or related field required; a Bachelor’s Degree in Public Health, Social Work, or related field preferred. Also, one to three years experience in outreach activities for a community health or social service agency; or any equivalent combination of training or experience which provides the following knowledge, abilities, and skills:

  • Knowledge of chronic and acute disease prevention strategies
  • Knowledge of New Haven’s social service network and community health programs
  • Ability to interact with diverse populations
  • Ability to facilitate group programs
  • Ability to express oneself verbally and in writing
  • Ability to maintain records, utilize a database, and prepare reports
  • Ability to work as part of a team
  • Ability to establish relationships and work effectively with community partners
  • Ability to advocate for clients and to link them to services
  • Ability to maintain clients’ confidentiality
  • Proficiency with Microsoft Office Suite
  • Bi-lingual / Bi-cultural in Spanish, preferred

NECESSARY SPECIAL REQUIREMENTS

  • Must possess and maintain a valid CT drivers license
  • Ability to drive the Mobile Outreach Van.

SALARY, BARGAINING UNIT & TERMS OF EMPLOYMENT

This is a "Special Fund," non-tested position. Continued employment contingent upon availability of future funding.

This position is included in a collective bargaining agreement with Local 3144, American Federation of State, County and Municipal Employees.

Special Funded employees shall not be covered by the City pension but shall continue to be covered by Social Security.

Employment is contingent upon the successful completion of: (1) a pre-employment background check, (2) a pre-employment physical examination, including pre-employment drug screening in accordance with the City’s Drug Free Workplace Policy*, and (3) a 90-day probationary period. Pursuant to the Federal Drug Free Workplace Act of 1988, The City of New Haven has a adopted a Drug Free Workplace Policy. The City of New Haven requires a pre-employment drug test, which includes screening for marijuana.
